Objective: Through the discussion of The treatment according to syndrome differentiation,drug-used characteristics and academic thought in the treatment of Hepatitis B Cirrhosis with Portal Hypertension of the Professor Lan Qingqiang,so as to summarize the clinical experience.Based on the splenic blood theory as the guiding ideology,to observe the curative effect of Guipi Decoction for basic Decoction drug for the treatment of the disease in patients with cirrhotic portal hypertension bleeding tendency,in order to provide new ideas for clinical treatment of the disease.Methods: The 80 cases were selected with certain diagnostic standard from Department of digestive diseases and liver diseases in the xx University And Affiliated Hospital.The 80 cases randomly assigned to the control group and the treatment group,40 cases in each group.The control group was treated with propranolol,treatment group was treated with Guipi decoction,a total of 6 months of treatment.Before and after treatment,the symptoms,signs,liver function,liver fibrosis index,coagulation function,degree of esophageal and gastric varices and Abdominal ultrasound were observed and compared between the two groups.Results:(1)clinical efficacy: the total effective rate of the control group was75%,the total effective rate of the treatment group was 84.21%,the total effective rate of the treatment group was higher than that of the control group,the difference was statistically significant(P<0.05).(2)TCM syndrome score:TCM syndrome scores were lower after treatment in the two groups,the difference was statistically significant(P<0.05);the two groups after treatment compared to the treatment group than the control group(P < 0.05),that Guipi Decoction can improve the patients with TCM syndrome score.(3)liver function: two groups of cases after treatment ALT?AST?TBIL?ALB have different degrees of improvement(P < 0.05),and the treatment group is better than the control group(P < 0.05).After two groups of treatment,GLOB increased(P>0.05),and the difference was not statistically significant.(4)liver fibrosis index: two groups of patients after treatment,liver fibrosis indicators HA?LN?IV –C?PC-III were better than before treatment(P < 0.05),the difference was statistically significant.After treatment,the treatment group was better than the control group(P < 0.05),and the difference was statistically significant.(5)prothrombin time?prothrombin activity: after treatment,prothrombin time?prothrombin activity in two groups was better than that before treatment(P<0.05),and the treatment group was better than the control group(P<0.05),the difference was statistically significant.(6)platelet count: after treatment,platelet count increased in the two groups(P<0.05),and the treatment group was better than the control group(P<0.05),the difference was statistically significant.(7)the degree of esophageal and gastric varices: the degree of esophageal varices in the esophagus of the two groups was significantly improved after treatment(P < 0.05).Compared with the control group,the treatment group was better than the control group(P < 0.05).(8)abdominal B ultrasound: after treatment,the diameter of portal vein,the length of spleen and the thickness of spleen were measured by B-mode ultrasound.The two groups of patients were significantly improved(P<0.05),compared with the control group,P<0.05,had significant difference.Conclusion: Professor Lan Qingqiang based on The Theory of The Spleen Controlling Blood treating patients in Hepatitis B Cirrhosis with Portal Hypertension Bleeding Tendency by using Guipi decoction can significantly improve the clinical symptoms,liver function,liver fibrosis,blood coagulation,the degree of esophageal and gastric varices and abdominal ultrasound of the patiens after treatment.which has a certain guiding significance for the clinical treatment of liver cirrhosis with portal hypertension.
