Study On The Effects Of Using Yuan Huan Tong Managing Platform To Implement Self-management Intervention For Stable COPD Patients

ObjectiveThe objective of this research was to develop the Yuan Huan Tong manageing platform based on WeChat,to explore the effects of self-management intervention implemented through this platform on the self-management ability,life quality,health literacy level and exercise ability of chronic obstructive pulmonary disease(COPD)patients in stable stage.MethodsThis is quasi-experiment recruited 94 stable COPD patients from three communities in Changsha city.Participants were randomly assigned to either the experimental group(n=47)or the control group(n=47).The researchers provided "the COPD patients managing guide",implemented personalized self-management intervention plan to the patients in both groups.The control group received regular telephone follow-up once a month.The experimental group was managed through the Yuan Huan Tong managing platform,filled in health diary every day,received disease related knowledge message every week,and received the disease related assessment scales every month,and the patients could communicate with researchers at any time by the platform.Before and after 3 months intervention,patients of the two groups were measured lung function(before intervention only)and 6 minutes walk test and under the guidance of the researchers filled the COPD self management scale,St Georges Respiratory Questionnaire(SGRQ)and Chronic Obstructive Pulmonary Disease knowledge Questionnaire to assesse patients ' lung function,exercise capacity,self-management ability,life quality and health literacy level of COPD patients.All the data were analyzed using SPSS 20.0 statistical software.The statistical methods included descriptive statistics,chi-square test,t test and nonparametric test.And p<0.05 was statistically significant.Results1.After the intervention the scores of self management scale and the Chronic Obstructive Pulmonary Disease knowledge Questionnaire in the experimental group were significantly higher than that of the control group(p<0.05),the score of symptom part of St Georges respiratory questionnaire was significantly lower than that of the control group(p<0.05);The scores of the total scale,activities part and influence part of St Georges's respiratory questionnaire and the results of 6 minutes walk test showed no significant difference(p>0.05)between the two groups after the intervention.2.After intervention the scores of the total scale,life management dimension,symptom management dimension,self-efficacy dimension and emotion management dimension of the self management scale,and the Chronic Obstructive Pulmonary Disease knowledge Questionnaire were higher than that before the intervention in the control group,the difference was statistically significant(p<0.05);the scores of the information management dimension in self management scale,St Georges respiratory questionnaire and 6 minutes walk test results showed no significant difference(p>0.05)copared to that before the intervention in the control group.3.After intervention the scores of self management scale and Chronic Obstructive Pulmonary Disease knowledge Questionnaire were significantly higher than that before the intervention in the experimental group,the difference was statistically significant(p<0.05);the scores of St Georges's respiratory questionnaire and 6 minute walk test results showed no significant difference(p>0.05)copared to that before the intervention in the experimental group.Conclustions1.Self-management intervention implemented through the Yuan Huan Tong managing platform can improve the self management ability and health literacy level of COPD patients in the stable stage.2.The effect of self-management intervention implemented through the Yuan Huan Tong managing platform on the life quality of stable COPD patients is limited,which can improve the cough,expectoration,dyspnea and other symptoms of COPD patients.
