ObjectivesTo discuss the feasibility of nurse parctitioners’ management mode,we worked out the plan for COPD patients in order to provide comprehensive, standardized and individualized rehabilitation nursing service system.To explore the NP management influence of quality of life in patients with COPD.MethodsCombined with China’s health care system and medical care health resources in this region and applied in the process of rehabilitation of patients with COPD, we worked out the plan for COPD patients after consulting from experts and reading a large number of relevant literature, taking example from foreign advanced care physician management methods. To randomize the 80 patients with COPD were hospitalized in Dong’e People’s Hospital from March 2013 to August 2013. The NP management group implemented hospital comprehensive rehabilitation nursing and community track rehabilitation guidance. The other group gave the routine treatments.After the patients discharged from the hospital, we did a six-month follow-up.The two groups of patients were measured pulmonary function, quality of life(SGRQ), exercise endurance(BODE index),the overall average hospital patient satisfaction, average medical expenses, readmission rate on admission and at discharge, 3 months and 6 months after discharge.Information was collected and typed into the EXCEL8.0,and was analyzed by using SPSS 17.0 software.Results:1.Two groups of patients with general information, pulmonary function, SGRQ scores, BODE index comparison, there were no statistically significant difference when admission.(P > 0.05)2.The indicators comparison after discharge: lung function index of intervention group patients was obviously higher than that of control group(P < 0.05); In comparison of SGRQ score results, the intervention group patients SGRQ total score and component(symptoms, activities, influence points) score were lower than control group(P < 0.05); at the same level The BODE index results comparison, in addition to BMI index(P > 0.05), other indicators were statistically significant(P < 0.05).The indicators of patients satisfaction survey in the intervention group was better than control group, with statistical significance(P < 0.05).3.The comparison of the indicators after 3-month and 6-month discharge:Lung function index of intervention group patients was obviously higher than that of control group(P<0.05).In SGRQ score results comparison, the intervention group patients SGRQ total score and each component(symptoms, activities, influence points) score were lower than control group(P<0.05).At the same level,the BODE index results comparison, the intervention group was better than control group, the indicators have statistical significance(P< 0.05). In the indicators of patients satisfaction survey in the intervention group was better than control group, with statistical significance(P< 0.05). Intervention group patients within six months from admission to discharge,the average medical expenses, average such confinement and readmission rate were lower than that of control group with statistical significance(P<0.05).Conclusions:Through the pulmonary rehabilitation education, the NP management of COPD can decrease the symptom of dispnea,increase the activity ability,enhance activity endurance, improve nutritional status, effectively improve the patient’s satisfaction and the quality of life, and reduce the patients’ average medical expenses, average, readmission rate.
