Diabetes mellitus is a serious chronic disease that threatens human health. The incidence of diabetes is increasing year by year, both in developed and developing countries. In nearly 20 years China's prevalence of diabetes increased by 4 times. As it has the world's second highest number of patients, making the situation of diabetes prevention and control quite grim in China. The health management as a new concept of health services is very important for comprehensive prevention and control of chronic diseases as well as community health services. Considering China's national conditions, establishing a health management in community that can provide guidance in areas such as diabetes knowledge, behavior, diet, medication methods, sports, etc, take continuous and effective monitoring as well as intervention, help patients adjust the lifestyle to the best state from aspects of physical, psychological, social adaptability is of unignorable significance.
