The Study On The Influence Factors Of Community Health Services On Residents Of City And Rural Region, Taishan Area

Objective:(1)To learn the health status and the major influence factors to residents' health. (2) To study the demand and utilization of health services. (3) To understand the utilization of community health services. (4) To explore the resident's willings and understanding for community health services. (5) To analyze the factors which effect on residents'community health services demand and utilization and to make a scientific basis to the health reform.Methods:(1)Investigation object:To adopt a stratified random cluster sampling method to select the investigation object. To sample three villages and towns in Taishan area at random which were daimiao office, caiyuan office and shengzhuang town.Then from the three offices, to sample nine villages or streets at random. All residents of the nine villages were investigated.(2)Statistics method:Analysis of T test,Chi-Square test and Logistic regression were used.Results:(1)656 families, that is 2137 residents are investigated.One family has 3.3 people.The sex ratio(male:female) is 0.94:1.The proportion over 65 years old people is 7.3% and it gets up to the standard that of international aging.Illiteracy,half of illliteracy and elementary school accounts for 24.3% of all the surveyed people. (2)The latter two weeks prevalence rate of the investigated residents is 134.82%o.Among them the city is 140.86‰and the rural is127.69‰.The top five diseases are upper respiratory infection, hypertention, tracheitis,cerbral infarction diabetes mellitus and coronary heart disease. (3)The chronic illness prevalence rate of which is108.11‰.Among them the city is 109.77%o and the rural is 106.19‰The top five diseases are hypertention, coronary heart disease, chronic bronchitis, cerbral infarction and diabetes mellitus. (4)The latter two weeks rate of seeing a doctor is 120.33%o.Among them the city is 121.51‰and the rural is 118.84%o. The rate of people to be hospitalized every year is 17.92%o and the rural is 3.76‰。(5) The rate of a prenatal examination is 100% both in city and rural lying-in woman. The rate of a inoculating below 2 yeas old children is 100%. (6)The proportion of the city residents to see a doctor in outpatient service, a clinic or a health service center is 59.1% and the rural residents is 57.2%. The outpatient services utilization of community health services is better. The proportion of the city residents to be hospitalized in villages and towns health center or community hospital is 4.2% and the rural residents is 33.3%. (7) The outpatient health service need is 2542759.13 person-time and the outpatient health service demand is 2161345.26 person-time. The family sickbed health service need is 42227.77 person-bed-time and the health service demand is 35893.60. person-bed-time (8)The major influence factors to residents'health are a bad habit in lifestyle such as smoking,drinking and lacking of physical training, economy factors and social medical security model.Conclusion:The work of community health services in taishan area has obtained some achievements,but it need to be sustainable development. Taian community health services must focus on aged population, women and children nursing and chronic disease management to overall develop in prevention, medical treatment, health care, recovering to health, health education and family planning technical guidance.
