BackgroundTraditional Chinese medicine is a kind of medicine that is gradually developed and enriched by the Chinese people in the process of production,living,practical activities and struggle with diseases.As an excellent traditional culture of the Chinese nation,Traditional Chinese medicine culture has made a very outstanding contribution to the prosperity of the Chinese nation.Giving full play to the advantages of traditional Chinese medicine's characteristic services and promoting the development of traditional Chinese medicine health services are an inevitable requirement for the comprehensive development of traditional Chinese medicine in an all-round way.It is also an important task for promoting the development of the national health service industry.It will deepen the reform of traditional China's medical and health system,change the way of national economic development,and improve the health quality of the whole people.The quality of health and the transformation of national economic development methods are of great significance.With the continuous deepening of traditional China's medical and health reform,the primary health care institutions have been playing a more and more important role in medical services as a key investment and support project for medical reform at all levels of government.Traditional Chinese medicine has achieved remarkable results in the development of primary medical health care institutions.Primary health care institutions are the main positions of traditional Chinese medicine services and the foundation for the survival and development of traditional Chinese medicine businesses.Transferring the focus of traditional Chinese medicine work to primary medical health care institutions is an important guarantee and important task for the sustainable development of traditional Chinese medicine.However,there are still many problems that restrict the development of traditional Chinese medicine.For example,traditional Chinese medicine service can not adapt to the growing health needs of the people,the advantages of traditional Chinese medicine gradually fade away,the field of traditional Chinese medicine service is shrinking,the infrastructure of the development of traditional Chinese medicine is poor,and the medical personnel of traditional Chinese medicine are scarce.In addition,from the study of the equity of the allocation of Chinese medicine health resources,there is no research on the equity of the allocation of traditional Chinese medicine health resources abroad.The domestic research on the equity of the allocation of traditional Chinese medicine health resources is mainly concentrated in the hospital of traditional Chinese medicine,most of which are the characteristics of the allocation of health resources in traditional Chinese medicine hospitals and their configuration.The equity analysis and evaluation of some kind of health resource allocation in traditional Chinese medicine hospital and the influencing factors on the equity of health resources allocation in traditional Chinese medicine hospitals are studied.There are few related studies on the equity of the allocation of traditional Chinese medicine health resources in the primary health care institutions,and the research is mainly focused on the equity of the staffing of traditional Chinese medicine,and it is short of the grass-roots level.Primary health care institutions,traditional Chinese medicine health human resources,traditional Chinese medicine health material resources and traditional Chinese medicine health finance resources comprehensive system research.ObjectiveThis study assessed the existing problems in the distribution of traditional Chinese medicine health resources in primary health care institutions through a systematic evaluation of the status quo and equity of the allocation of health resources for traditional Chinese medicine in Shandong Province,and the policy recommendations are put forward in order to promote the sustainability of the medical and health resources in the primary health care institutions,providing reference for the sustainable development of medical and health resources.1.To analyze the status quo of health resource allocation of traditional Chinese medicine in primary health care institutions in Shandong Province,and to find out the shortcomings and problems in the allocation of health resources for traditional Chinese medicine in primary health care institutions;2.Quantitatively evaluate the equity of the distribution of traditional Chinese medicine health resources in the primary health care institutions in Shandong Province;3.Based on the problems found in this study,we propose policy recommendations for optimizing the allocation of traditional Chinese medicine health resources in primary health care institutions.MethodsIn this study,we selected 1,692 township hospitals(including community health service centers)that could provide health services for primary health care institutions in Shandong Province and used this as the research object of the thesis.The relevant data and study of this research mainly comes from the "Inspection and evaluation of the project of improving the capacity of the basic traditional Chinese medicine service and the "national medical Hall of the central health center" by the Shandong provincial health and Family Planning Commission and the Shandong Provincial Administration of traditional traditional Chinese medicine.1.Descriptive statistical analysis methods:Descriptive statistical analysis is used to analyze the status quo of health resource allocation of traditional Chinese medicine in the primary health care institutions in Shandong Province,and to find out the shortcomings and problems in the process of allocation;2.Equity analysis method:Using the Gini coefficient,the Theil index and the concentration index three analysis methods to analyze and evaluate the equity of the allocation of traditional Chinese medicine health resources in the primary health institutions of Shandong province.Research Results1.The distribution of health resources for traditional Chinese medicine in primary health care institutions in Shandong ProvinceBy 2016,the population of the 26-45 year old age group for traditional Chinese medicine health human resources in the primary health care institutions was the largest,accounting for about 60%of the total population;The ratio of senior,intermediate and junior titles was 6.65:6.34:1;The number of college graduates and below was the main,accounting for more than 50%of the total population,accounting for more than 50%of the total number of people;The proportion of traditional Chinese medicines per 1,000 population in medical care has generally reached 1:0.34;Traditional Chinese medicine health material resources have shown an increasing trend;Among them,the area of traditional Chinese medicine business houses has been growing fastest;The income of per capita traditional Chinese medicine special subsidy has declined year by year.2.Results of the equity analysis of traditional Chinese medicine health resources in primary health care institutions in Shandong ProvinceThe results of equity in the allocation of traditional Chinese medicine health human resources and traditional Chinese medicine health and material resources in 2014 and 2016 are as follows:Traditional Chinese medicine practitioners,registered nurses,pharmacists and general practitioners of traditional Chinese medicine are ranked according to the Gini coefficient of population distribution.The value of 0.25,the geographical distribution of the Gini coefficient of the maximum value of 0.30,are less than the equity line 0.40;Chinese medicine health human resources,the overall population index of the total population distribution is 0.07,according to the geographical distribution of the maximum value of 0.10,the contribution rate of the theil index in the region is higher than that of the interregional;Traditional Chinese medicine practitioners,registered nurses,pharmacists,and general practitioners in the Chinese medicine category focus on the minimum index value of-0.07 and the maximum value of 0.01;The maximum value of Gini coefficient of resources according to population distribution is 0.16,and the maximum value of geographical Gini coefficient is 0.20,which is less than the standard of the fair line 0.40;the maximum value of the population distribution of Taier index according to population is 0.03.The geographical distribution of the maximum value is 0.06,the contribution rate in the region is greater than the interregional contribution rate;The maximum concentration index of traditional Chinese medicine health resources is-0.01.Conclusions and SuggestionsThe structure of the distribution of traditional Chinese medicine health human resources in primary health care institutions in Shandong Province is irrational,and there are significant differences in the specific budgets for traditional Chinese medicine.In addition,the unfairness in the allocation of traditional Chinese medicine health resources in primary health care institutions is mainly caused by regional differences.The distribution of health resources for traditional Chinese medicine in regions with good economic development level is rather poor.Therefore,it is recommended that the health administrative department should optimize the allocation of traditional Chinese medicine health resources at the primary health care institutions in accordance with the actual situation of the allocation of traditional Chinese medicine health resources,improve the allocation system of traditional Chinese medicine health human resources,pay attention to the construction of nursing teams,and raise funds through multiple channels.In addition to the financial investment in the field of traditional Chinese medicine and health,it is necessary to fully consider various factors to formulate plans to optimize the allocation of traditional Chinese health resources.
