| Objective:To analyze the clinical data of 70 cases of occult organism-origin chemical pneumonia(cryptogenic tissue-type pneumonia),summarize the clinical features,imaging and pathological features,prognosis and prognosis of COP,so as to improve the clinician's C-type Understanding of pneumonia.Clinical diagnosis and treatment level.Methods: Retrospectively analyzed the clinical data of 70 patients with COP diagnosed by pathologists and joint clinicians First Affiliated Hospital of Wannan Medical College from July 2014 to January 2017.Their clinical characteristics and chest CT images were analyzed.Changes,routine biochemistry,immune-related tests,treatment,and outcomes were analyzed statistically and comprehensively.Results:Of the 70 patients,39 were males and 31 were females,aged 18 to 86 years,mean(62±13)years;5 patients had diabetes mellitus,10 had hypertension,and 9 had coronary heart disease.The main clinical symptoms are cough,shortness of breath,fever,chest tightness,chest pain,neck and back pain and other respiratory system performance,and a few have hemoptysis,night sweats and other symptoms.Chest CT manifestations are diverse,with multiple real lung changes,nodular signs,and isolated focal shadows.Pathological morphology consistent with the performance of mechanistic pneumonia.Sixty-two patients treated with glucocorticoids were followed up for 3 to 36 months(median,20 months).Their clinical symptoms and signs improved,imaging lesions absorbed significantly less than before hospitalization,and56 patients completely recovered.3 cases had recurrence,and 2 of 70 cases had acuteexacerbations.The condition was stabilized after increasing the dose of hormone.Conclusion:The clinical manifestations of COP are not typical.The imaging examination lacks specificity and is often misdiagnosed as other pulmonary diseases.The diagnosis mainly relies on pathology and imaging and needs to exclude other secondary diseases.The effect of glucocorticoid therapy is better. |
