Chronic kidney disease(CKD)is one of the third most common cause of death in dog.In recent years,proteinuria is considered as an independent predictor in the process of CKD.Proteinuria decreasing can slow down the progression of CKD,prolong the survival time and maybe improve the life quality.At present,as the trend of aging in companion animals,CKD will become one of the most common diseases in older animals,which is worthy to get further study.CKD is a long-term result from any chronic stimulation to the kidneys,which eventually lead to glomerular and tubular injury.Constantly negative effects on kidneys due to the compensatory irreversible results,such as glomerular and renal tubular structural changes,functional damages,and significant proteinuria.Nevertheless,current available choice of treatment in proteinuria in CKD is limited,such as angiotensin converting enzyme inhibitors(ACEIs).Angiotensin II receptor 1 type blockers(ARBs)are the antagonists to adverse the effects of angiotensin II receptor 1 and have the same renoprotective effect like ACEIs.Telmisartan is a new ARBs and it offers another option to deal with proteinuria in CKD in dogs.In fact,there is few study of the efficacy and benefits of telmisartan in the treatment of chronic nephrotic proteinuria in dogs.Therefore,this trial attempts to observe the clinical efficacy of chronic nephrotic proteinuria treating with telmisartan.This trial is conducive to further understanding and improve the treatment of proteinuria in CKD,in the meanwhile,may provide a theoretical basis for clinical therapy and enriches the plan in treating proteinuria in CKD.Test I Establishment of proteinuria in chronic kidney disease model on dogsIn this trial,oral administration of adenine for 28 days in dogs to establishment.In this period,clinical sings,blood,urine samples from dogs were collected.Statistical analysis shown that dogs appeared clinical symptoms which were matched with chronic nephrotic proteinuria.Also,results from histopathological biopsy slides observation were consisted with chronic kidney disease's pathological changes.Successfully animal model of chronic renal nephrotic proteinuria has built.Test ? Efficacy observation of treatment with telmisartan on proteinuria in chronic kidney disease in dogsIn this study,dogs with proteinuric chronic kidney disease were treated with telmisartan for 14 days.By observing the clinical signs of the animals,compared with the non-treatment group,telmisartan could improve the performance of the animals(the response to the sounds and animals' vigilance,increased appetite,etc.).At the same time,by statistical analyzing the blood and urine samples,telmisartan shows a positive effect for improving CKD associated with significant proteinuria:the kidney index returned to normal range,relieved azotemic syndrome,rebalanced the electrolyte disorder.It is important that telmisartan has begin to show a significant clinical efficacy in the treatment in CKD with severe proteinuria after continuous treatment within 1 week.The results revealed that continuous administration of telmisartan within 1 week can significantly reduced proteinuria in CKD;continuous administration for 14 days showed that clinical symptoms and animal quality of life have improved significantly.The animals shown well tolerated.This trail's outcome are consistent with the results of the current limited study:the antiproteinuria effect in CKD in dogs with telmisartan is safe and effective.
