Located around the megacity Wuhan,in the process of intense urbanization,the problem of unbalanced supply and demand of medical service facilities in small towns in Wuhan remains prominent.Combining with the state to further deepen the medical and health reform requirements for basic medical facilities during in the 13 th Five-Year Plan period,according to the goal of the optimization of basic medical facilities,achieving fairness in urban and rural areas,and improving the efficiency of the use of facilities,this article introduces the concept of supply and demand coordination,uses the coupling coordination model,analyzes of the coordination level and coordination type of the current supply and demand relationship of medical services in small towns,then advices on the optimization of configuration of main medical service facilities(township clinic)in small towns.The main content of this article is divided into three parts:The first part is an analysis of the status quo of supply of medical service facilities in small towns in Wuhan,and then summarizes the supply characteristics and problems.The first is to sort out the development history and development characteristics of small towns in Wuhan since the reform and opening up.Due to historical development and policy guidance,small towns within and outside urban development have developed very different development characteristics.In addition,the history of the development of medical and health services in Wuhan was combed,and the hospitals gradually formed a system and gradually improved along with this process.In terms of policy,although Wuhan has recently issued relevant policies and plans,emphasizing the importance of promoting grading diagnosis and strengthening primary medical care,the attention of small towns is still relatively low.The allocation standards are mainly divided according to administrative levels and service radius,and have less consideration for residents' needs.Although facilities and service capabilities of medical service facilities in small towns are improving,form a clear circle structure and hierarchy on the spatial distribution,and regional cooperation is also initially realized,the following issues still stand out: the unbalanced distribution of medical facilities between urban and rural areas,the imbalance in medical service capacity among small towns,the shortage of medical resources and waste at the time of mismatch between supply and demand,the failure to highlight the hierarchical nature of facilities,and the incomplete sharing of regional facilities,etc.The second part focuses on the analysis of the demand for medical service facilities in small towns and summarizes the general types of demand patterns and characteristics of medical services.The factors influence the medical needs of residents in small towns in Wuhan are divided into the demand body-based factors,the facility-object factors,and the institutional factors in service acquisition.In terms of the demand body-based factors,the residents' age structure,social class,and health status will produce differentiated medical needs.As far as the facility-object factors is concerned,the level of the medical service facility itself will affect the satisfaction of residents and determine their medical behavior.With regard to institutional factors,the accessibility of medical facilities and medical insurance policies will also affect residents' willingness to seek treatment.The medical needs of residents of small towns also have obvious spatial circle characteristics.The third part proposes the concept of coordination of supply and demand of medical services,and analyzes the purpose and connotation of supply and demand coordination.At the same time,according to the factors that can represent supply and demand levels,an assessment index system for supply and demand coordination is constructed.Then introduce a coupling coordination model to evaluate the relationship between supply and demand,and the degree of coordinated development.The evaluation results show that the overall supply and demand relationship is coordinated but the coordination level is lower.The small towns with uncoordinated supply and demand often fall In a state of insufficiency.The supply and demand relationship also shows significant circle characteristics.According to the relationship of supply and demand demonstrated by each small town,from these aspects of optimization of circle functions,the optimization of multi-supply structure,the optimization of response demands,and the optimization of regional coordination policies,optimize the configuration of its medical service facilities...
