The influence of human factor to the safety of nuclear power plant becomes more and more important recently along with the increasement of bussiness operating of nuclear power plants.However,human error analysis in every human reliability analysis(HRA)tool has different kinds of shortcomings,most of them are lack of human behavior analysis and qualitative analysis capability,and the others are lack of cause analysis and are difficult to use.Thus,it is necessary to develop human error mode and causes for human error analysis,and combing Failure Modes,Effects and Criticality Analysis(FMECA)method provides a way to induce human error modes and causes to reach the requirement of human error analysis.FMECA is a reliability analysis method widely used in industry,which induces failure modes,causes and effects of one product,and provides severity and criticality of every failure modes.FMECA can provide accurate identification of failure modes,which is accepted by industry and has become a reliability process which has to be completed during the development of a system.The combination of related elements from FMECA and HRA methods generates a new HRA method called Human Error Mode,Effects and Criticality Analysis(HEMECA)method.Based on advanced cognitive reliability analysis,HEMECA has advantages from FMECA which can identify human modes,analyze related cause and influence,as well as provides the severity and criticality of human error mode.Following work has been done within this paper to create HEMECA method.First,based on the investigation of important concepts of FMECA and human error analysis,this paper analyzes the disadvantages of existing HRA methods,and basic elements concept of HEMECA is defined by comparing similar concepts from FMECA and HRA,such as failure mode and human error mode,root cause and performance shaping factor(PSF).And the feasibility of HEMECA is clarified.Second,based on the discussion of evaluation criteria of HRA method,the HEMECA method of nuclear power plant is constructed.The human error mode list is formed by new method and each mode is associated with several relevant PSF,which forms a human error mode analyzing system easy to operate.At the same time,the severity and criticality level is defined,the output table is designed and anlaysis process is developed.Third,using the HEMECA method built above,three typical human factor events from national nuclear safety association in 2016,Three Mile Island severe accident and anticipated severe accidents of HPR1000 are analyzed.The result show that the trace analysis of human factor events and Three Mile Island is in good agreement with result from reports.Predictive analysis of HPR1000 can provide optimize suggestions for designing power plants.HEMECA method basically meets the requirements of HRA qualitative analysis.
