Objective: To compare the effects of three intervention methods on the indexes of glucose and lipid metabolism,physical health and abnormal plantar pressure,so as to understand the effects of these intervention schemes on improving the physical health of type 2 diabetic patients and preventing and delaying the occurrence of complications To analyze the mechanism of action and provide reference for the prevention and treatment of type 2 diabetes.Methods: 61 type 2 diabetic patients with vitamin D deficiency were randomly divided into vitamin D group(n = 14),aerobic exercise group(n = 15),vitamin D combined with aerobic exercise group(n = 18)and control group(n = 14).The vitamin D group received 1000 IU of vitamin D every day for 3 consecutive months;the aerobic exercise group used moderate intensity aerobic exercise,50 minutes / time,3 times a week,for 3 consecutive months;the vitamin D combined with aerobic exercise group intervened in the way of vitamin D + aerobic exercise;the control group kept the original lifestyle unchanged.Four groups of patients did not intervene with the original hypoglycemic drugs.The physical health(body weight,BMI,waist circumference,hip circumference,body fat rate,muscle volume,fasting blood glucose,glycosylated hemoglobin),plantar pressure(peak pressure,percentage of pressure load in front,middle and back of foot),static balance stability(track length of gravity center movement,range of gravity Center shaking)and other indicators were detected before and after the intervention.SPSS 18.0 statistical software package was used to analyze the data.Paired sample t-test was used to test the changes in the group before and after itself.Repeated measurement ANOVA was used between groups.Multivariate ANOVA was used to analyze the interaction between the two intervention methods.P < 0.05 was the significant level,P < 0.01 was the non constant significant level.Result:(1)Fasting blood glucose and glycosylated hemoglobin: after 3 months of intervention,the blood glucose of the patients in vitamin D group decreased significantly(P Significant difference(P > 0.05).Comparison between groups:compared with the control group,the glycosylated hemoglobin of vitamin D combined with aerobic exercise group was significantly lower(P < 0.05).The interaction between the two intervention methods on fasting blood glucose was significant(P < 0.05),there was antagonistic effect;the two intervention methods had no significant interaction on glycosylated hemoglobin(P > 0.05).(2)Physical indicators: after intervention,the weight of patients in aerobic exercise group decreased significantly(P < 0.05),but there was no significant difference in the other three groups(P > 0.05);BMI in vitamin D combined with aerobic exercise group decreased significantly(P < 0.05),BMI in the other three groups decreased in different degrees,but there was no significant change(P > 0.05);waist hip ratio After intervention,except for the decrease of vitamin D combined with aerobic exercise group,the other three groups all increased in varying degrees,but there was no significant difference among the four groups(P > 0.05).Compared with the control group,the waist circumference of vitamin D combined with aerobic exercise group decreased significantly(P < 0.05).(3)Foot pressure index:(1)peak pressure: after 3 months of intervention,the peak pressure of 4 groups decreased to different degrees,especially in vitamin D group(P < 0.05);compared with the control group,the peak pressure of left foot decreased significantly(P < 0.05);(2)percentage of pressure load: after 3 months of intervention,compared with the aerobic exercise group,vitamin D combined with In the oxygen exercise group,the percentage of pressure load in the middle of left foot decreased significantly(P < 0.05).(4)Static balance stability index:(1)track length of gravity center movement:after 3 months of intervention,the track length of gravity center movement in the vitamin D combined with aerobic exercise group was significantly reduced when the left foot was standing on one leg(P < 0.05);when the eyes were closed,the track length of gravity center movement in the three intervention groups was significantly reduced when the left foot was standing on one leg(P < 0.05).(2)Amplitude range of gravity center shaking: after 3 months of intervention,the amplitude range of gravity center shaking in three intervention groups decreased significantly(P There was interaction in the condition of standing with left foot closed eyes(P < 0.01)and standing with right foot closed eyes(P < 0.05).The two intervention methods had antagonistic effect on the above indexes.Conclusion:(1)Three months of low intensity aerobic exercise combined with vitamin D supplementation can reduce the level of glycosylated hemoglobin in patients with type2 diabetes,but there is no synergistic effect between the two interventions.(2)Through three months of intervention,vitamin D supplementation or aerobic exercise intervention can reduce the body weight and BMI of type 2 diabetes patients,but there is no synergistic effect between the two interventions.(3)Taking vitamin D can reduce the abnormal increase of plantar pressure in type 2 diabetic patients to a certain extent.(4)After 3 months of intervention,vitamin D or aerobic exercise can enhance the balance stability of patients,but there is no synergistic effect.
