Wu Yu's Clinical Dose-effect Relationship And Application Characteristics Research

Purpose:To study on the data of the main diseases,symptoms,dosages and its combination herbs by establishing the database of Evodia formulae,and using data mining methods,and to explore the clinical application rules of Evodia from the perspective of toxicity,efficacy and dosage.To summarize the clinical dosage and application characteristic of Evodia with descr iptive statistics method,in order to provide the guidance for clinical safe and effective use of Evodia,and also to lay the foundation for the literature research of it.Method:1 Data resourceWith the search words"wuzhuyu","wuyu and "shizhuyu",this study searched the liter atures about the clinical experience,case report and clinical observation of the Chinese medicine application of Evodia between ten years from January 1,1957 to December 31,2017 on CNKI,and there were 648 formulae after selection.In addition,745 for mulae were selected from the ancient literatures listed in ancient literatuers.2 Literature research methodTo retrieve the records related to the toxicity and efficacy of Evodia in different dynasties,and to ex plore the influencing factors3 Data miningInput the data of the selected formulae into Excel on Windows 7,and then output the data and mining it with SPSS 17.00.Results:In decoction,the overall average value of Evodia dosage is 17.32g,the common dosage in all ages is 1.12g-18.65g.it's much less in Qing dynasty,1.12g-3.73g,which is comparatively close to the dosage of Evodia in decoction approved by Chinese Pharmacopoeia 2010 edition,which is 2g-5g.In powders,the general average value of Evodia dosage is 27.10g,and the range of its common dosage in all ages is 3.73g-41.30g.There was a big fluctuation after Song dynasty(except for Jin-Yuan dynasty),for example,in Ming dynasty,the dosage of Evodia ranged from 0.37g to 261.50g,the data was comparatively scattered,and the dosage recorded in many powder formulae was for one-time processing,or for the integral dosage of one course of treatment,which cannot reflect thoroughly the real dosage of Evodia in all the dynasties.The diseases mainly treated by Evodia in ancient times including:masses,deficient-cold,febrile disease,gynecological problem,post-natal diseases,coldness,barbiers,chest apoplexy,wind evil,cholera,asthenic diseases,phlegm-dampness,dysphagia syndrome,hernia,etc.The symptoms mainly treated by Evodia in ancient times including:ache,distention and ful lness,diarrhea,vomit,feeling depressed,vomiting and diarrhea,acid regurgitation,qi-gushing sensation,syncope collapse,cough,poor digestion,metrorrhagia and metrostaxis,mo rbid leucorrhea,spasm,poor appetite,etc.the ache includes stomachache,heart pain,febrile p ain,stomachache,post-natal pain,hypochondriac pain and foot pain.The dosage of Evodia used by ancient doctors was different in treating different diseases,symptoms,disease conditions and physical constitutions.For example,the dosage is comparatively small in treating masses,deficient-cold,post-natal diseases and deficient type diseases,ache,distention and fullness,diarrhea,feeling depressed,metrorrhagia and metrostaxis.etc.while the dosage is comparatively big when treating cholera,coldness,febrile disease,wind caused diseases,qi-gushing sensation,syncope collapse,cough,etc.The main compatibility of Evodia in ancient medical records in ancient.imes including:1.Evodia combines with Cortex Cinnamomi;2.Evodia combines with Radix Angelicae Sinensis;3.Evodia combines with Fresh Ginger;4.Evodia combines with Radix et Rhizoma Glycyrrhizae;5.Evodia combines with Pericarpium Citri Reticulatae;6.Evodia combines with Ginseng;7.Evodia combines with Dry Ginger;8.Evodia combines with Poria;9.Evodia combines with Rhizoma Pinelliae;10.Evodia combines with Cortex Magnoliae Officinalis;11.Evodia combines with Radix Aucklandiae;12.Evodia combines with Rhizoma Atractylodis Macrocephalae;13.Evodia combines with Semen Arecae;14.Evodia combines with Radix Paeoniae Alba;15.Evodia combines with Rhizoma Chuanxiong;The diseases mainly treated by Evodia currently including:Chronic Gastritis,Prosper Meniere,Neuropathic headache,chronic colitis,high blood pressure,cephalagia,superficial gastritis,duodenal ulcer,reflux esophagitis.The use of prescription for modern doctors,Evodia's commonly used range of 9g-12g,of which Evodia is mainly involved in:10g(24.2%),6g(16.7%),15g(13.1%)and so on.Mainly treated by Evodia currently including:headache,stomachache,nausea and vomiting,dizzy,diarrhea,dysmenorrhea,abdominal pain,distention and fullness,acid regurgitation,hypochondriac pain,Bi-syndrome,pernicious vomiting,hiccup,asthma.Chest obstruction.In the current medical record of clinical application,the most common dosage of Evodia is 10g.6g and 15g.Among the formulae with Evodia in the medical records,the Evodia decoction is the most frequently used,which is 421 times,and the average dosage of Evodia is 10.67g,the range of its common dosage is 9g-12g.Channel Warming Decoction has been used 38 times,and the average dosage of Evodia is 8.97g,the range of its common dosage is 6g-10g.Chinese Angelica Frigid Extremities Decoction combined with Evodia Ginger Decoction has been used 36 times,and the average dosage of Evodia is 9.3 Og,the range of its common dosage is 6g-10g.Four Spirits Pill has been used 32 times,and the average dosage of Evodia is 8.59g,the range of its common dosage is 6g-10g,its main treatment includes abdominal pain and diarrhea.Left Metal Pill has been used 22 times,,and the average dosage of Evodia is 4.72g,the range of its common dosage is 2.37g-6g.It is obvious that the dosage of Evodia is different in different formulae.The main compatibility of Evodia in current medical records in ancient times including:1.Evodia combines with Fresh Ginger;2.Evodia combines with Jujubae;3.Evodia combines with Codonopsis pilosula;4.Evodia combines with Radix et Rhizoma Glycyrrhizae;5.Evodia combines with Pinellia ternata;6.Evodia combines with Radices Paeoniae Alba;7.Evodia combines with Angelica;8.Evodia combines with Cassia Twig;9.Evodia combines with Bighead Atractylodes Rhizome;10.Evodia combines with Poria Cocos;11.Evodia combines with Ligusticum wallichii;12.Evodia combines with Dried Tangerine;13.Evodia combines with Rhizoma Zingiberis;14.Evodia combines with Asarum Sieboldi Mig;15.Evodia combines with Ginseng;ConclusionThe toxicity and effect of Evodia are determined by its properties.Its main function including warming the middle and dispelling cold,descending counteflow of qi to stop vomiting,strengthening yang to relieve diarrhea.Its function features as acrid and warm to relieving cold and bitter and dryness to descending counteflow of qi,which is also the main character of its toxicity.Activating fire,consuming yin(fluid,blood),injuring prenatal qi are the main mechanisms of its toxicity,which may cause alopecia,unclear eyesight,blurred vision,eye sores,obstruction on pharynx and larynx,illusion,chest distention,etc.Adjustment of the relationship of "toxicity and effect" of Evodia is the key to maximize the effectiveness of Evodia in clinical application.On the basic of disease differentiation and syndrome differentiation,moderate dosage,proper combination and strict processing are the key methods for the safe and effective use of Evodia.
