Analysis Of The Use Of Xingnaojing Based On Medical Data And The Role Of Clinical Pharmacists In Traditional Chinese Medicine

Objective:To promote the rational clinical use of Xingnaojing Injection and provide the reference for Clinic pharmacist of traditional Chinese Medicine to make pharmaceutical care program of Traditional Chinese Medicine injection,which mainly based on the investigation of rationality and safety of Xingnaojing Injection on the treatment for acute cerebral infarction patients.Method:Collecting the patients who using Xingnaojing Injection because of acute cerebral infarction from the electronic medical records of the third grade specialized hospital between September 2016 and January 2017.Collecting the personal information of the patients,the disease,the use of Xingnaojing Injection and the adverse events and so on.And analyzing the rationality and safety of Xingnaojing Injection on clinical application according to clinical guidelines,literature and demographic data,the consciousness of the patients,GCS score,time of application of Xingnaojing Injection and dosage,solvent,course of treatment and adverse events.Result1 Case collectionThe study investigated 550 cases who use Xingnaojing Injection after admission,and 196 cases diagnosed as acute cerebral infarction were enrolled.2 Demography surveyAcute cerebral infarction mainly occurs in elderly people and women.Of all the investigated cases,16 cases died,which associated with more serious complications,including lung infection.3 Disease surveyAmong the investigated cases,39.09%of the patients with acute cerebral infarction had recurred.-The Chinese doctors did not test the related inflammatory factors in the blood of the patients who enrolled the group,and doctor’s description of the patient’s consciousness who use Xingnaojing Injection was chaotic.The cases of GCS coma score accounted for 56.63%,and some cases had no indication of the use of Xingnaojing Injection.Patients with acute cerebral infarction and occurred more than 3 times in have a increasing proportion of disturbance of consciousness.4 The use of Xingnaojing InjectionIn the application of Xingnaojing Injection,4 cases used with a time interval of more than 14 days;the route of administration for all the investigated cases were intravenous drip;and there were off-label use of Xingnaojing Injection including dissolving medium,single dose and frequency of administration,and there was 1 case both used Xingnaojing Injection and potassium chloride;2 cases were treated with Xingnaojing Injection over 28 days.Xingnaojing Injection also combined with western medicine in clinical application,and the top twenty drugs were antiplatelet aggregation drug,brain protective agent,defecation drug,antilipidemic drug,expectorant antitussive antiasthmatic drug and dehydrating drug.The combination of Xingnaojing Injection,and traditional Chinese medicine:the traditional Chinese medicine mainly focused on blood-regulating recipes;there were 21 cases with syndrome differentiation,among which 20 cases were phlegm heat syndrome and 1 cases of yang hyperactivity syndrome.5 Adverse event1 case had anaphylactic dermatitis during the use of Xingnaojing Injection and 2 cases had diarrhoea;20 cases of 1 days of medical records did not mention the reason of withdrawal of Xingnaojing Injection in medical records,of which 3 cases were mentioned with epilepsy or seizures.Conclusion:Through the study of 196 cases with acute cerebral infarction and analysis of the relevant data,the conclusions were drawn as follows:1 There are many unreasonable problems exist in clinical use of Xingnaojing Injection,including overdosage,unreasonable solvent selection,compatibility infusion with Western medicine,long course of treatment.What is worse,the clinical use of Xingnaojing Injection sometime without syndrome differentiation2 Xingnaojing Injection belong to the cold-cool nature drug,may has side effects such as diarrhea.Moreover,attention should be paid on the patients with epilepsy who use Xingnaojing Injection.3 Clinical Pharmacists of Chinese Medicine should pay more attention to pharmaceutical care including dosage,solvent and so on to help doctors use Xingnaojing Injection on clinical rationally.4 By monitoring the signs change during the whole process clinical usage of traditional Chinese medicine Xingnaojing Injection,Clinical pharmacists can provide assistance to physicians in the rational use of Xingnaojing Injection and ensure the safety of clinical rational use.The study evaluated the rationality and safety of Xingnaojing Injection uses on clinical through investigation,summarize the monitoring notes of Xingnaojing Injection during the use,and provide the reference for Clinic pharmacist of traditional Chinese Medicine to make pharmaceutical care program of Traditional Chinese Medicine injection.
