Background:Knee Osteoarthritis(KOA)is a common chronic degenerative joint disease,which mainly encroaches on the articular cartilage,synovium,joint capsule and surrounding muscles.Its clinical features are recurrent and aggravating joint swelling and pain,dyskinesia and even joint deformities,which seriously affect the mental health level and quality of life of patients.Therefore,it is of great significance to study KOA's impact on patients' QoL and to explore the effect of chronic disease management.Objective:To investigate and evaluate the QoL,anxiety and depression condition of KOA patients by questionnaire,and to analyze the possible influencing factors about it.Through intervening KOA patients with chronic disease management measures,to observe and investigate its effects on KOA patients' disease control and QoL.Research contents:1 Literature reviews:They discuss KOA's epidemiology,the etiology and pathogenesis,integrated traditional Chinese and Western treatment,as well as the progress of chronic disease management of KOA at home and abroad.2 Clinical research2.1 A survey of KOA's QoLMethods:It used SF-36,SAS,SDS scale to investigate and evaluate the QoL,anxiety and depression condition of KOA patients,with analyzing the possible influencing factors about it.Results:?Conpared with general population of our country,KOA patients' score in the 8 dimensions of SF-36 scale was lower(P<0.05).?While the SAS and SDS scores were higher(P<0.05).?In the investigation of a number of related factors,gender,the payment method of medical expenses,the frequency of outpatient visits and the place of residence all had influenc in KOA's QoL(P<0.05).2.2 Effects of chronic disease management on disease control and QoL in KOA.Methods:Trough a randomized controlled trial design,64 cases of KOA who meet the inclusion criteria were divided into the treatment group and the control group,with 32 cases in each group.The treatment group was given the chronic disease management measures based on accepted conventional therapy on the TCM,while the control group received only conventional therapy of traditional Chinese medicine,without chronic disease management measures.The treatment course is 12 weeks.The SF-36 score,SAS score,SDS score and WOMAC score of the two groups of KOA patients were observed before and after the treatment.Results:?The two groups' scores both improved after treatment.?Compared with the control group,the SF-36 score,SAS score,SDS score and WOMAC score of the treatment group were significantly improved after the intervention of chronic disease management(P<0.05).Conclusion:?The level of KOA patients'QoL is lower than the general population,involved in many aspects including physiology and psychology,and KOA patients are more likely to suffer from anxiety,depression and other negative psychological state.?Gender,the payment method of medical expenses,the frequency of outpatient visits and the place of residence are related to it.?The conventional therapy on the TCM can impove KOA patients' QoL.while the effect of chronic disease management is more significance.?The intervention measures of chronic disease management to KOA patients is beneficial to control their condition.It can also improves the the anxiety and depression of KOA,and improves the quality of life of KOA patients.
