Research background:Chronic Kidney Disease is now a high incidence of disease,modern medicine often chooses immunosuppressive drugs such as cytotoxic drugs,some can achieve certain effect,but at the same time these could bring a lot of adverse reactions,and often relapse after therapy.Progressively,there are currently no definitive drugs can prevent renal impairment.Animal experiments and clinical experiments have proven that Chinese medicine can indeed delay the progression of kidney damage.The theory of fu-xie in traditional Chinese medicine has been fairly mature through the development of doctors in different dynasties.It has been used to solve some difficult medical problems.Therefore,There may be some certain advantages in the treatment of Chronic Kidney Disease under the guidance of theory of fu-xie.Object:Summarize the thoughts and methods of the tutor applying the theory of fu-xie to treat kidney disease,observe and evaluate the efficacy of the therapy of kidney disease of the patients guided by the theory of fu-xie,and observe the therapeutic effect,thereby confirming the correctness of the theory of fu-xie and the guiding significance of it in clinical therapy of the kidney.Methods:Using real-world self-control trials,with the tutor's guidance of the theory of fu-xie,63 patients were given oral Chinese medicine for 1 month to 6 months.Data were taken at 1st,3rd,6th month respectively.The 24hUTP,Scr,clinical symptoms,and adverse reactions were recorded.The routine blood test,routine urine test,liver function,and other safety indicators were also observed.According to the Guidelines for Clinical Research of New Chinese Medicines,we made the criterion of the curative effect,and statistical methods such as paired sample t-test and non-parametric test were used to analyze the data,and assess the efficacy of Chinese medicine.Result:(1)The analysis of the laboratory results of 63 patients:after Chinese medicine therapy,the effective rate(in terms of Scr)at the 1st,3rd,6th month were 69.0%,87.0%and 81.5%respectively,Scr of 23 patients who completed 6 months of therapy were down at the 1st,3rd month,the difference was statistically significant compared with therapy before(P<0.05,P<0.01);eGFR was increased at the 3 points of follow-up after therapy and at the 3rd and 6th month,there were statistically significant difference compared with therapy before(P<0.01,P<0.01);24hUTP was decreased at each follow-up point compared with therapy before,and the difference was statistically significant at 6th month compared with therapy before.(2)The effective rate(in terms of 24hUTP)of CKD 1-2 patients were 76.2%,83.3%,and 84.6%at 1st,3rd,6th month respectively.The 24hUTP was lower than therapy before after 6 months' treatment in 11 patients.The difference at the 3rd and 6th month were statistically significant than therapy before(P<0.01,P<0.01);Scr and eGFR were getting batter in the 8 patients after treatment at the 3rd and 6th month than therapy before.There were statistically difference at 3rd and 6th month(P<0.01,P<0.05)respectively.The effective rate(in terms of Scr)of CKD 3-5 patients was 81.8%,88.9%,and 80.0%at 1st,3rd,6th month respectively.The Scr of 13 patients with a 6-month course of treatment decreased compared with that before treatment,there were statistically difference at 1st and 3rd month(P<0.01,P<0.05).At the same time,eGFR increased compared with that before therapy,and the difference was statistically significant at 1st and 3rd month compared with that before treatment(P<0.01;P<0.05).(3)The different effect of the 21 cases before and after Chinese medicine treatment:The 21 patients had been treated with Western medicine for at least 3 months before Chinese medicine treatment,during the 3 months,Scr gradually increased,eGFR gradually decreased,followed by Chinese medicine treatment.After 1 month of Chinese medicine treatment,Scr decreased compared with that before therapy,the difference was statistically significant(P<0.05),eGFR increased,the difference was not statistically significant;when the time of therapy was up to 3 months,16 patients were treated with traditional Chinese medicine.Scr gradually increased in the 3 months of Western medicine treatment,while decreased during Chinese medicine treatment,the difference were statistically significant compared with that before treatment(P<0.01,P<0.01),the change of eGFR is the opposite.(4)The results of IgAN showed that after 6-month-course therapy of Chinese medicine,Scr decreased compared with that before therapy,especially at the 3rd month,then Scr increased slightly from the 3rd month to the 6th month,but still lower than that before therapy.After 3-month-course therapy,eGFR increased,the difference was statistically significant compared with that before therapy(P<0.01).The 24 hUTP was decreasing overall,but there was no statistically significant difference compared with that before therapy.Conclusion:The treatment effect of patients with CKD under the guidance of the theory of fu-xie is indeed effective.The indicators of patients were getting better,including Scr,eGFR,24hUTP.Traditional Chinese medicine can reduce proteinuria,protect the kidneys,and delay the progression of kidney damage.On the whole,the treatment of Liu Jing program is safe and effective,and there is no obvious adverse reactions.The application of the theory of fu-xie to kidney disease is worthy of promotion and further study.
