Changes Of TCM Syndromes Of Cold And Heat In 60 Patients With Advanced Non-small Cell Lung Cancer Before And After Paclitaxel Plus Cisplatin Chemotherapy

[Objective]Prospective research methods were used to investigate the composition of cold and heat syndrome before and after chemotherapy in patients with advanced non-small cell lung cancer treated with paclitaxel and cisplatin.The evolution of cold and heat syndrome before and after chemotherapy was explored to provide an objective decision for decision making in the process of syndrome differentiation and treatment.The basis is to improve the clinical efficacy of combined treatment of lung cancer with traditional Chinese and Western medicine,and to better exert the advantages of traditional Chinese medicine in reducing toxicity and increasing efficiency in the course of chemotherapy.[Methods]Sixty-three patients with advanced non-small cell lung cancer(NSCLC)who met the inclusion criteria were selected and their TCM syndrome information was collected.The cold and hot Chinese medicine was classified according to the standard of Chinese cold and heat syndrome differentiation.One of the patients was asked to withdraw from the study for personal reasons.Two patients were lost.Finally,60 patients completed the clinical observation.The self-designed TCM Symptom Collection Form(see Appendix 6 for details)was used to collect the general conditions of the patients,pre-chemotherapy syndrome information,and chemotherapy with paclitaxel and cisplatin,with 1 cycle for 21 days and 1 week after 2 cycles of chemotherapy.Collect the syndrome information again.The TCM syndrome differentiation type is determined by a senior professional TCM researcher who is responsible for the completion of the reference to the appendix.Record data and perform statistical analysis.[Results]1.A total of 60 patients with advanced non-small cell lung cancer were enrolled in this study.The results of a comprehensive clinical observation study showed that 60 patients had common symptoms of cough,hemoptysis,sputum,chest tightness/chest pain,fatigue,dry mouth,and dry throat.Fever,dry stool,do not want to drink water,cold,loose stools,nausea and vomiting,hair loss,muscle and joint pain,limb sensation of dull/abnormal.After TP regimen chemotherapy,the symptoms of cough,hemoptysis,sputum,chest tightness/chest pain,and fatigue and fatigue were generally relieved.Obviously cold symptoms:I do not know the taste(do not want to drink water),cold,loose stools symptoms before the increase,apparently a hot symptoms of dry mouth and throat,fever,dry stool was significantly reduced compared with the previous,and have statistical difference.The greatest increases in chemotherapy after TP regimen were nausea and vomiting,alopecia,muscle and joint,and limb sensation of dullness/abnormal symptoms.All four symptoms were common adverse reactions of the TP regimen.After the TP regimen chemotherapy,the pale-tongue tongue,dim/bright tongue,thin white fur,and white thick coating of multi-dominant cold syndrome increased more than before,and the red/dark yellow greasy moss and thin yellow fur of multiple main heat syndrome decreased compared with before chemotherapy and had statistical difference.2.After chemotherapy with TP regimen,11 cases(34.4%)of 32 heat syndrome patients were converted from heat syndrome before treatment to cold syndrome,and 28 cold syndrome patients remained cold syndrome after treatment.The change before and after was significant(P=0.01).It is suggested that the TP regimen chemotherapy has a certain influence on cold and hot Chinese medicine in patients with lung cancer,and can make the patient turn from cold syndrome to cold syndrome.3.In the aspect of adverse reactions,more adverse reactions after chemotherapy by TP regimen were nausea and vomiting(56.7%),alopecia(71.7%),muscle and joint pain(65%),and limb sensation/aberration(25%).Chemotherapy drugs for the evil drug,easy to injure the spleen and stomach,spleen and stomach transport disorders,lifting loss Division,stomach gas caused by adverse nausea and vomiting.The toxic side effects of chemotherapy drugs affect the function of the spleen and stomach and the metaplasia of qi and blood,and eventually will lead to liver without blood to hide,and the kidney will suffer from azoospermia.As a result,deficiency of essential blood can not raise hair and form hair loss.The muscle pain that occurs after chemotherapy in the TP regimen can be attributed to the"hysteria" of traditional Chinese medicine,which is an important cause of snoring.In the study,we also observed that most of the patients with muscle and joint pain were not accompanied by redness,burning,and other thermal syndromes.Most of the neurologically retarded/abnormal sensations experienced cold worsening.The neuromuscular pain and limb sensation/abnormality after TP regimen were more likely to be cold syndromes.[Conclusion]The TP regimen can affect the patient's cold and heat syndromes,increase the symptoms of the patient's tendency to coldness,increase the number of tongue images,and tend to reduce the symptoms of fever and tongue.After chemotherapeutic treatment with TP regimen,the patients with cold syndrome increased more than before,and those with heat syndrome decreased compared with before.34.3%of patients with heat syndrome turned cold syndrome,and all had statistical differences.The number of cases in this study is small,yet further large-sample,multi-center,long-term follow-up clinical studies are needed to provide more forceful data,so as to provide a basis for better exerting the role of Chinese medicine in reducing toxicity and increasing efficacy during chemotherapy.
