Background:In recent years,with the development of the bio-psychological-medical model,research on the mental health of diabetic patients has received extensive attention.Current epidemiological data show that at least 30%patients with diabetes are associated with clinically significant depressive disorders,of which type 2 diabetes is the main cause of diabetes.Some research reports show that the prevalence of diabetes patients with depression is between 24%and 30%.Faced with the increasing prevalence of tension,Chinese medicine practitioners currently have little research on them and are still in their infancy.They have not yet established a unified The dialectic standard system lacks the correct method of judging objectively the efficacy.Therefore,the research on the syndrome distribution of the disease is imminent.Depression status is a kind of "occult depression".Depression is often covered by somatic symptoms and is easily misdiagnosed and missed.As a result,patients with type 2 diabetes and depressive symptoms are unable to obtain timely correct diagnosis and effective treatment,resulting in prolonged healing.To aggravate the condition,it is urgent to improve the recognition ability of type 2 diabetes with depression,enhance clinical diagnostic ability,and reduce the suffering of patients.Objective:Through self-made questionnaires,basic information,basic clinical conditions,and signs and symptoms of patients with type 2 diabetes were collected,including age,sex,marital status,occupation,education level,height,and weight.Then the SDS scale was used to diagnose the degree of depression and depression in type 2 diabetes mellitus,and the cluster method was used to explore the TCM syndrome types of type 2 diabetes mellitus with depression.Methods:Using SPSS22.0 software,the frequency of descriptive analysis of the general condition of type 2 diabetes with depression,the main symptoms were counted using the frequency method,and the system clusters their The results were classified,and the comparison method was used to compare the contents of the "Practical Chinese Internal Medicine","Clinical Guidelines for New Drugs of Traditional Chinese Medicine" and "National Standards for Clinical Diagnosis and Treatment of TCM Syndromes" to explore the classification of medical syndromes.Results:1.Basic patient information55.2%of women with type 2 diabetes mellitus and depression were eligible for inclusion in this study.Men accounted for 44.8%.Among 116 patients,the maximum,minimum,and average ages were 82 years and 29 years,respectively.At 56.8± 13.302 years of age,the education level is mainly concentrated in junior colleges and junior high schools.Marriage accounts for the largest proportion of married people,which is 78.4%.Divorced and widowed are less.2.Basic clinical conditions of patientsin the case of medical expenses,a total of 25.8%of patients were found to have spent more than 20%of the total family income on diabetes treatment.but in this study,only 2 patients undergoing antidepressant treatment and none had undergone psychotherapy.In the investigation of 116 patients related clinical conditions,the mean duration of diabetes was 11.34±10.2 years,78.4%of patients thought their blood glucose control was normal,and 76 patients had complications,accounting for 65.5%of the total number of patients.49.1%of the patients reported that they had normal sleep quality.In this study,65.5%of the patients were mild depression,29.3%were moderate depression,and 5,2%were major depression.BMI,FPG and HbAcl were positively correlated with SDS scores.3.Patient's symptoms and signsThis study collected the symptoms and signs of type 2 diabetes with depression and used it as a variable to analyze the data using systematic clustering.The study showed that the top ten most frequent symptoms were:insomnia,upset and annoyance,shortness of breath,laziness,thirst,lack of energy,palpitation,depression,hunger,bloating,and chest fullness.The most common tongue tongue with less red tongue is 84.48%.The first three positions of the pulse frequency distribution are pulse,pulse string and pulse number,and the percentage is more than 40%.4.Type 2 diabetes mellitus with depression distributionAccording to the results of cluster analysis,type 2 diabetes with depression was divided into four syndrome types:Qi and Yin deficiency,liver depression and spleen deficiency,yin and yang deficiency,and blood stasis.Conclusions:1.BMI,FPG and HbAcl were positively correlated with SDS scores.2.According to the results of cluster analysis,based on TCM syndrome differentiation theory,referring to the diagnostic criteria of TCM syndromes,combined with the actual clinical conditions,under the guidance of the expert group,type 2 diabetes with depression is divided into four syndrome types:.Qi and Yin deficiency,Liver depression spleen deficiency,yin and yang deficiency,blood stasis blocking network.
