Objective1.To investigate theabnormalities characteristic of spontaneous brain activityin type2 diabetes mellitus with and without depressive state by Regional homogeneity(Re Ho)based on resting-state f MRI.2.To illustrate the clinical parameters effect on abnormal brain activities andfurther understand the comorbidity disease for impairment of cognitive function.Materials and Methods In present study,38 cases were screened by PHQ-9 for diabetes patients with depressive state(DM-D),as well as 41/28 gender-,age-,edu-matched diabetes mellitus(DM)and healthy subjects(CON)were selected,which were examined by the United States3.0T resting-state f MRI scanner.Based on the platform of Matlab 14.0,abnormal brain activities in three groups were compared by Re Ho method usingone-way ANOVA and post-ANOVA.The Pearson correlation analysis and multiple regression analysis were used to analysis the correlation between abnormal activitybrain regions and clinical and neuropsychological scale parameters.Results1.After one-way ANOVA,three group showed consistency of significantly increased in frontal lobe,temporal lobe,occipital lobe,cerebellum,namely Re Ho values inthose brain areas were obvious enhanced.This means neurons activities significant increased in those localized brain regions.2.Compared with the CON group,DM-D group showed decreased Re Ho values in the parietal lobe,right frontal orbit Lobe,left middle frontal gyrus,left occipital Lobe/cuneus/calcarine,left superior temporal gyrus,while increased in the frontal gyrus,right superior frontal orbit Lobe,right frontal gyrus subcallosal gyrus /lentiform nucleus/olfactory,left superior/ middle /inferior frontal gyrus/insula.3.Compared with the CON group,DM group showed decreased Re Ho values in the bilateral middle occipital gyrus,bilateral cingulate gyrus,right cerebellum posterior lobe,right medial frontal gyrus,left occipital lobe /fusiform gyrus/lingual gyrus,while increased in the right superior temporal gyrus,right anterior cingulated,left inferior parietal lobule/angular gyrus,bilateral frontal gyrus.4.Compared with the DM group,DM-D group showed increased in the right superior orbit medial frontal Lobe,left inferior frontal gyrus/putamen/insula/lentiformnucleus/subcallosal gyrus,righ middle occipital gyrus/cuneus/calcarine,and significantly decreased Re Ho value mainly in the righ inferior parietal lobule / angular gyrus/supra marginal,bilateral parietal lobe postcentral gyrus/supra marginal,leftparietal lobe / precuneus/angular,right middle temporal gyrus,left occipital lobe /cuneus.5.In the DM-D group,Pearson correlation between the scores of neuropsychologyscales and abnormal brain activity regions showed,(1).the enhanced ReHo values of the left frontal orbital positively correlated with PHQ-9 and the AVLT(R=0.436,P=0.007;R=0.324,P=0.047).(2).The reduced Re Ho values in the left precuneus negatively correlated with PHQ-9(R=-0.371,P=0.022),and positively correlated with the DSST(R=0.351,P=0.031).In the right middle temporal gyrus is positively correlatedwith the AVLT(R=0.322,P=0.049),in the left angular gyrus was negatively correlatedwith TMT-B(R=-0.357,P=0.028),while positively correlated with the AVLT(R=0.361,P=0.026).The multiple regression analysis showed the Re Ho values increased in the left frontal orbital gyrus,Hb A1C(R=0.459,P=0.004)and diastolic blood pressure(R=0.351,P=0.031)were most impact,the decreased Re Ho values in left precuneuswas HOME-IR(R=0.351,P=0.031),in left angular gyrus were Hb A1C(R=0.353,P=0.030)and course(R=0.351,P=0.031),in right middle temporal gyrus were HOME-IR(R=1.333,P=0.000)?Hb A1C(R=1.232,P=0.000).Conclusion1.Multiple spontaneous brain areas in type 2 diabetes mellitus with andwithout depressive state,which is a great signification to understand diabetes mellitus wasan important risk factor for brain dysfunction.2.Compared to a single diabetes,combine with depression statepatients showed the correlation between significant abnormal brain activity regions and neural cognition scales revealed the potential neurophysiological mechanisms for cognitive impairment.And for non dementia combine depression state patients,f MRI is a useful means of inspection for detect and diagnosis early changes of brain function and cognitive impairment.3.Multiple regression analysis results showed Insulin resistance index(HOME-IR),glycosylated hemoglobin(HbA1C),course of disease were the independent risk factorsfor the combine depression state patients,which suggested improving insulin resistance,effective control blood glucose may help relieved depression,even correctedbrain regions abnormal activity and ultimately improve the cognitive decline.
