Correlation Between Rheumatoid Arthritis And TCM Constitution Types

Objective:(1)To explore the Traditional Chinese Medicine constitution distribution regularity on patents with rheumatoid arthritis,whether exist constitutional susceptibility or not;(2)To discuss the factors of different Traditional Chinese Medicine among rheumatoid arthritis patients;(3)To discuss the objective index differences among different Traditional Chinese Medicine constitution.To apply Traditional Chinese Medicine constitution classification to patients with rheumatoid arthritis,so as to provide new treatment direction and a reference basis for further clinical work.Methods: The study was a cross-sectional study.A total of 190 patients who met the inclusion criteria were selected as the subjects.We conducted all subjects to form general situation questionnaire and “Chinese constitution classification and judge” self-testing table,as well as selected clinical-related information,including joint involvement,inflammation index,immune index,joint blood flow signal classification and blood routine index.Then using statistical software to analysis the data,explore the patients' characteristics among different Traditional Chinese Medicine constitution and the correlation between Traditional Chinese Medicine constitution and rheumatoid arthritis.Using Excel software to log the data,and then organize the data and make statistical analysis using SPSS 20.0 statistical software.Statistical methods include descriptive statistical analysis,chi-square test,one-way ANOVA and rank sum test.Results: 1.General information of patients with rheumatoid arthritis: These patients showed a single type or mixed type(possessing two or more than two types).The patients with rheumatoid arthritis susceptible Traditional Chinese Medicine constitution is XU,including Yang deficiency?Yin deficiency?Qi deficiency,and the number of patients with Yang deficiency is the most.Some of the patients had more than 2 constitutions,the rate was 62.1%.2.From the result,we could see the differences on general information and clinical manifestation.Among different constitutions,there existed different age distribution(P?0.05).There were differences between Blood stasis and Yang deficiency?Phlegm dampness?damp-heat?Specific endowment.All the patients in the group of blood stasis were female.Among all the four age groups,Yang deficiency had the largest proportion;among the groups of age60-70 and over 70,Yin deficiency and Blood stasis had the larger proportion.In the group of over 70 years,Qi deficiency had the lower proportion.In the term of Phlegm dampness,the group of less than 50 years had the largest proportion.There were significant differences in the course of disease(p=0.007).The course of the disease was longer than the others,10.4±8.43 years,followed by damp-heat and Yin deficiency,9.49±11.76 and 8.6±7.27 years respectively.Among all the constitutions,there were no significant differences in the part of hypertension,diabetes and interstitial lung disease(p?0.05).3.Patients with different constitutions showed different subjective clinical indicators.There were significant differences among different constitutions on bone density.The bone density of Yang deficiency was the lowest,followed by Blood stasis,damp-heat and Yin deficiency.In the term of inflammation index,there was significant difference on D-dimer among different constitutions(the value of Phlegm dampness and Qi deficiency was higher than others),and there were no significant differences on the other index.In the term of immune index,there were significant differences on rheumatoid factors and ANA.For rheumatoid factors,the value of damp-heat was the highest,while the value of Yin deficiency and Specific endowment were lower relatively.In the part of ANA,there were significant differences between Yang deficiency and Yin deficiency ?Qi Stagnation,the proportion of ANA1:100 were lower,while the proportion in the group of Yang deficiency was the most.In the term of blood routine index,there was significant difference on hemoglobin among different constitution.The value of Specific endowment was high relatively,and that of Qi Stagnation was the lowest.There were no significant differences on joint ultrasonic blood flow signal classification.Conclusion:1.Rheumatoid arthritis exist susceptible constitution,which are Deficiency constitutions,while Yang deficiency is the most.2.There was significant difference on susceptible sex?age and the course of disease among different constitutions.3.There was significant difference on bone density among different constitutions.4.There was significant difference on D-dimer among different constitutions,while Phlegm dampness and Qi Stagnation were higher relatively.5.There were significant differences on rheumatoid factors and the positive rate of ANA among different constitutions.6.There was no significant difference on joint manifestation ? erythrocyte sedimentation rate?CRP? serum ferritin,AKA,APF,platelet and other laboratory examination results?having a history of chronic diseases or not?joint ultrasound among different constitutions.Traditional Chinese Medicine constitution could be regarded as a new treatment target for rheumatoid arthritis,as it is very easy to operate,so it is worthy of promoting and implementation.The study provided reference in the field of rheumatoid arthritis Traditional Chinese Medicine constitution,and I hope to explore the other correlations between Traditional Chinese Medicine constitution and rheumatoid arthritis in order to provide more guidance in the future.
