The Tenderness Response Of The Twelve Acupoints Of Tinnitus Patients And Its Correlation With The Characteristics Of Syndromes

1.Objective:Based on the observation of the twelve primary acupoints of tinnitus patients and the control group of tinnitus,the pain response of the primary acupoints and the threshold of tenderness were studied.At the same time,through the tinnitus group of TCM syndrome differentiation,to explore the relationship between the twelve primary acupoints in tinnitus and TCM syndrome type,to provide an objective basis for the tinnitus meridian viscera dialectical,thus guiding the clinical practice.2.Methods:The syndromes of tinnitus group according to symptom,syndrome type and frequency of tinnitus group records;using acupressure to press the 90 tinnitus group and 30 cases of non tinnitus control group of twelve primary acupoints,recorded twelve primary acupoints and their tender response frequency;and the use of tenderness pain threshold pressure tester twelve primary acupoints;using SPSS 20.0 and SAS 9.3 of the twelve primary acupoints of the frequency and pressure pain threshold statistics.3.Results:3.1 Generally patients with tinnitus:3.1.1 Gender:There were 30 males(33.33%)and 60 females(66.67%)in the tinnitus group,and there were 16 males(53.33%)and 14 females patients(46.67%)in the control group.There was a significant difference(P<0.05)among women in the tinnitus group.There was no significant difference between male and female patients in the non tinnitus control group and in the male and female patients in the control group(P>0.05).3.1.2 Age:The age of the largest number of tinnitus group is 51~60 years old(33.33%),followed by 41?50 years old(21.11%);non tinnitus control group of the largest age is 51?60 years(43.33%),followed by 18?30 years of age(23.33%),the tinnitus tinnitus group and non control group in each age group showed no significant difference(P>0.05).3.1.3 Duration:Less than 3 months,29 cases(acute tinnitus),3?12 months,16 cases(subacute tinnitus),more than 12 months,45 cases(chronic tinnitus).More than 12 months of chronic tinnitus group was the most common,the incidence rate was 50%,and the number of acute tinnitus and subacute tinnitus,there was a significant difference(P<0.01).3.1.4 Tinnitus site:90 cases of tinnitus group,double tinnitus in 39 cases(43.33%),30 cases of left tinnitus(33.33%),19 cases of right tinnitus(21.11%),2 cases of brain(2.22%).Double tinnitus had the highest incidence,followed by the left lower incidence of tinnitus,tinnitus cerebri.The double number is the right number of tinnitus and tinnitus,there was significant difference(P<0.05),either left or right double tinnitus,tinnitus and tinnitus were buzzing in number,the difference had significant difference(P<0.01).There was no significant difference between the two groups(P>0.05).3.1.5 Tinnitus severity classification:Of the 90 cases,there were 41 cases(grade 2),grade II(45.56%),grade II(38.89%),grade III(10.00%),and grade IV(5.56%).The number of the first class cases was the highest,followed by the second grade,and the number of grade I and II was significantly higher than that of grade III and IV,there was a significant difference(P<0.01).·3.1.6 TCM Syndrome type:in 90 cases of tinnitus group,there were 27 kinds of syndrome types,one type of syndrome type,the compound type of the syndrome was 15.The number of patients appeared syndromes of the top five followed by hyperactivity of liver Yang in 17 cases,accounting for 18.89%;perturbationsuffered on 15 cases,accounting for 16.67%;and 7 cases of liver qi stagnation,kidney essence deficiency accounted for 7.78%;6 cases,accounting for 6.67%;5 cases of spleen and stomach,accounted for 5.56%.The rest of the patients were in 5 cases.In 90 cases of tinnitus group,there were five internal organs related syndromes,including liver related syndrome in 55 cases,accounting for 61.11%;spleen related syndrome in 18 cases,accounting for 20%;kidney related syndrome in 12 cases,accounting for 13.33%;the other two viscus related syndromes were less than 10 cases.3.1.7 Concomitant disease:Tinnitus patients with cervical spondylosis in 47 cases(52.22%),the control group with non tinnitus 11 cases of cervical spondylosis(36.67%);tinnitus group with high blood fat in 24 cases(26.67%),the control group of non tinnitus patients with hyperlipidemia in 4 cases(13.33%);22 cases of tinnitus group with hypertension(24.44%),the control group of non tinnitus patients with hypertension 4 cases(13.33%),in three with the disease,whether or not the tinnitus tinnitus group in the control group,cervical spondylosis were high blood lipids,high blood pressure,there was significant difference(P<0.05),but the comparison between tinnitus and tinnitus group and non control group,there was no significant difference(P>0.05).3.2 The cases of twelve primary acupoints tenderness:3.2.1 The cases of primary acupoints tenderness in tinnitus group:In 90 cases of tinnitus group,there were tenderness reactions in the twelve primary acupoints,and the frequency of tenderness reactions was sorted and clustered into five categories.First category:Taichong(LR3),Hegu(LI4),Qiuxu(GB40);second category:Taibai(SP3);third category:Taiyuan(LU9),Yangchi(SJ4),Taixi(KI3);Fourth category:Shenmen(HT7),Jinggu(BL64),Chongyang(ST42),Wangu(SI4);fifth category:Daling(PC7).Among the acupoints of frequency comparison results are as follows:(1)Taichong(LR3),Hegu(LI4)frequency were more than Taibai(SP3),Taiyuan(LU9),Yangchi(SJ4),Taixi(KI3),Shenmen(HT7),Jinggu(BL64),Chongyang(ST42),Wangu(SI4),Daling(PC7),there was significant difference(P<0.05);(2)Qiuxu(GB40)frequency was more than Taiyuan(LU9),Yangchi(SJ4),Taixi(KI3),Shenmen(HT7),Jinggu(BL64),Chongyang(ST42),Wangu(SI4),Daling(PC7),there was a significant difference(P<0.01);(3)Taibai(SP3)frequency was more than Taixi(KI3),Shenmen(HT7),Jinggu(BL64),Chongyang(ST42),Wangu(SI4),Daling(PC7),there was significant difference(P<0.05);(4)Taiyuan(LU9)frequency was more than Shenmen(HT7),Jinggu(BL64),Chongyang(ST42),Wangu(SI4),Daling(PC7),there was a significant difference(P<0.01);(5)Yangchi(SJ4)frequency was more than Shenmen(HT7),Jinggu(BL64),Chongyang(ST42),Wangu(SI4),Daling(PC7),there was significant difference(P<0.05);(6)Taixi(KI3)frequency was more than Chongyang(ST42),Wangu(SI4),Daling(PC7),there was significant difference(P<0.05);(7)Shenmen(HT7),Jinggu(BL64)frequency were more than Daling(PC7),there was significant difference(P<0.05).There was no significant difference between the rest(P>0.05).3.2.2 The cases of primary acupoints tenderness in control group:30 cases of control group of twelve primary acupoints also showed tenderness reaction,the tender points according to the frequency response from more to less sorting and clustering,clustered into four categories,the first category:Taichong(LR3),Hegu(LI4);second category:Qiuxu(GB40),Taibai(SP3),Taixi(KI3);third category:Taiyuan(LU9),Yangchi(SJ4),Chongyang(ST42);fourth category:Shenmen(HT7),Daling(PC7),Jinggu(BL64),Wangu(SI4).Among the acupoints of frequency comparison results are as follows:(1)Taichong(LR3),Hegu(LI4)frequency were more than Taibai(SP3),Taixi(KI3),Taiyuan(LU9),Yangchi(SJ4),Chongyang(ST42),Shenmen(HT7),Daling(PC7),Jinggu(BL64),Wangu(SI4),there was si gnificant difference(P<0.05);(2)Qiuxu(GB40)frequency were more than Yangchi(SJ4),Chongyang(ST42),Shenmen(HT7),Daling(PC7),Jinggu(BL64),Wangu(SI4)there was significant difference(P<0.05);(3)Taibai(SP3),Taixi(KI3)frequency were more than Shenmen(HT7),Daling(PC7),Jinggu(BL64),Wangu(SI4),there was significant difference(P<0.05);(4)Taiyuan(LU9)frequency was more than Jinggu(BL64),there were significant differences(P<0.05).There was no significant difference between the rest(P>0.05).3.2.3 Comparison of the frequency of tenderness between tinnitus group and control group:Compared with the control group,there was significant difference in Jinggu(BL64)between the tinnitus group and the control group(P<0.05),and there was no significant difference in other primary acupoints(P>0.05).3.3 The threshold of pain threshold in twelve primary acupoints:3.3.1 The threshold of twelve primary acupoints in tinnitus group:90 cases of tinnitus group twelve primary acupoints tenderness threshold from low to high is in the following order:Chongyang(ST42),Hegu(LI4),Taichong(LR3),Wangu(SI4),Yangchi(SJ4),Taiyuan(LU9),Taibai(SP3),Qiuxu(GB40),Shenmen(HT7),Jinggu(BL64),Daling(PC7),Taixi(KI3).Among the acupoints of comparison results are as follows:(1)The threshold of Chongyang(ST42),Hegu(LI4)were lower than that of Taichong(LR3),Wangu(SI4),Yangchi(SJ4),Taiyuan(LU9),Taibai(SP3),Qiuxu(GB40),Shenmen(HT7),Jinggu(BL64),Daling(PC7),Taixi(KI3),there was a significant difference(P<0.01);(2)The threshold of Taichong(LR3)was lower than Taibai(SP3),Qiuxu(GB40),Taichong(LR3),Shenmen(HT7),Jinggu(BL64),Daling(PC7),Taixi(KI3),there was significant difference(P<0.05);(3)The threshold of Wangu(SI4)was lower than Qiuxu(GB40),Shenmen(HT7),Jinggu(BL64),Daling(PC7),Taixi(KI3),there was a significant difference(P<0.01);(4)The threshold of Yangchi(SJ4),Taiyuan(LU9),Taibai(SP3),Qiuxu(GB40),Shenmen(HT7),Jinggu(BL64),Daling(PC7)were lower than Taixi(KI3),there was a significant difference(P<0.01);(5)The threshold of Shenmen(HT7)was lower than Taixi(KI3),there was a significant difference(P<0.01).There was no significant difference between the rest(P>0.05).3.3.2 The threshold of twelve primary acupoints in control group:30 cases of tinnitus group twelve primary acupoints tenderness threshold from low to high is in the following order:Wangu(SI4),Chongyang(ST42),Taichong(LR3),Hegu(LI4),Qiuxu(GB40),Taibai(SP3),Taiyuan(LU9),Yangchi(SJ4),Jinggu(BL64),Shenmen(HT7),Taixi(KI3),Daling(PC7).Among the acupoints of comparison results are as follows:(1)The threshold of Wangu(SI4),Chongyang(ST42)were lower than Taibai(SP3),Taiyuan(LU9),Y Yangchi(SJ4),Jinggu(BL64),Shenmen(HT7),Taixi(KI3),Daling(PC7),there was significant difference(P<0.05);(2)The threshold of Taichong(LR3)was lower than Taiyuan(LU9),Yangchi(SJ4),Jinggu(BL64),Shenmen(HT7),Taixi(KI3),Daling(PC7),there was significant difference(P<0.05);(3)The threshold of Qiuxu(GB40),Taibai(SP3),Taiyuan(LU9),Yangchi(SJ4)were lower than Jinggu(BL64),Shenmen(HT7),Taixi(KI3),Daling(PC7),there was significant difference(P<0.05);(4)The threshold of Jinggu(BL64),Shenmen(HT7)were lower than Daling(PC7),there was significant difference(P<0.05).There was no significant difference between the rest(P>0.05).3.3.3 Comparison of the twelve primary acupoints pain thresholds in tinnitus group and control group:The pressure pain threshold tinnitus group Taichong(LR3),Hegu(LI4),Taibai(SP3),Taiyuan(LU9),Yangchi(SJ4),Taixi(KI3),Shenmen(HT7),Jinggu(BL64),Chongyang(ST42)and Daling(PC7)are more sensitive to lower than that of control group,and the difference was statistically significant(P<0.05),compared with two points in the Wangu(SI4),Qiuxu(GB40)between tinnitus group and control group,the difference no statistical significance(P>0.05).3.4 The relationship between syndrome type and primary acupoints:Correlation of five syndromes of tinnitus group were mainly liver related syndromes,followed by spleen syndrome;at the same time,the liver spleen and stomach tenderness is relatively sensitive to the original point.4.Conclusions:4.1 Tinnitus group in women than men;age 51?60 years;the course of chronic tinnitus are;tinnitus side double tinnitus relatively more rare brain;tinnitus severity in grade I and II are;with underlying diseases,with cervical spondylosis of tinnitus influence.4.2 All twelve primary acupoints have tenderness reaction in tinnitus,and which is sensitive to Taichong(LR3),Hegu(LI4),Taibai(SP3),Taiyuan(LU9),Yangchi(SJ4),Taixi(KI3),Shenmen(HT7),Jinggu(BL64),Chongyang(ST42)and Daling(PC7).4.3 The main syndrome types of the five zang organs in the tinnitus group were liver related syndrome type,and the second was spleen related syndrome type.
