Clinical Pharmacists Of Traditional Chinese Medicine Participate In The Treatment Of Chronic Heart Failure With Integrated Chinese And Western Medicine

Chronic heart failure is the final destination of most cardiovascular disease,it is one of the main causes of death,the high incidence rate,high death rate,seriously affecting the quality of life of patients,is a major problem of modern medicine.CHF is a chronic heart failure due to any cardiac function or structure abnormal ejection or impaired ventricular filling a complex clinical syndrome.The main clinical manifestations were fatigue(limited exercise tolerance)and dyspnea,and fluid retention(peripheral edema and pulmonary congestion).Chronic heart failure(CHF)is one of the most important cardiovascular diseases in our country.According to the 42 hospitals in some areas of China,in the 10714 cases of heart failure patients retrospective study,we found that coronary heart disease ranks first in etiology,followed by hypertension;mortality rate of heart failure of all ages were higher than those of other cardiovascular disease,they are the main causes of death for left ventricular failure(59%),arrhythmia(13%)and sudden death(13%).For patients with chronic heart failure,improve their quality of life,reduce the mortality rate is one of the urgent problems.At present,the combination of Chinese and Western medicine treatment of chronic heart failure curative effect is remarkable,but refer to the 2014 China heart failure diagnosis and treatment guidelines need to use complex western medicine.Traditional Chinese medicine,Chinese medicine and other traditional Chinese medicine in chronic heart failure rank the role of more and more importantin the long-term treatment.In western medicine has definite curative effect on chronic heart failure,but the drugs used in the treatment process,also has many side effects,in addition to many pharmacological effects with prevention and treatment of heart failure.to ensure proper use of these drugs,patients with higher requirements on medical knowledge is necessary.once the improper use,it is prone to severe adverse drug reactions or medication error.WHO data show that 1/3 patients of the world's died of improper medication,the 1/7 patientsdid not die of natural disease,but died of irrational drug use.For example,the improper use of low molecular?inaccurate weight heparinand?irregularly test of the INR value may appear ecchymosis and subcutaneous hemorrhage.If the patient medication compliance is poor,the frequency of the use of these drugs will be greatly increased,the mortality rate of patients,the hospital cycle is also extended,the rate of readmission will improve.How to improve the survival rate of patients with chronic heart failure,reduce hospitalization period,reduce the rate of readmissionbecause of improper drug taking,become the clinical treatment of chronic heart failure imminent problem.The traditional Chinese medicine clinical pharmacy takes patients on as the service object,in order to adapt to the individual differences of different patients and complicated disease prevention,using modern pharmacy,traditional Chinese pharmacology and other professional knowledge combined with clinical situation,formulate a reasonable therapeutic regimen,monitoring process and exploring medication rules of patients,in order to ensure the clinical medication safe and effective.The traditional Chinese medicine pays attention to the combination of'disease""syndrome""medicine" and attaches importance to individual patients according to the etiology and pathogenesis of different drug regimens,embodies the deep foundation of clinical pharmacy of Chinese medicine,but also provides the basis for the development of clinical pharmacy of traditional Chinese medicine.The traditional Chinese medicine clinical pharmacy takes patients on as the service object,in order to adapt to the individual differences of different patients and complicated disease prevention,using modern pharmacy,traditional Chinese pharmacology and other professional pharmacy.The first part,exploration of medication management model that the clinical medicine of traditional Chinese medicine treats chronic heart failure.This part is divided into three parts:literature review?pharmacists ' management in the treatment of CHF and intervention research.The literature review part expounds the treatment and symptom of CHF in the calendar version of Chinese internal medicine,the symptoms of chronic heart failure and treatment were summarized,to provide reference for clinical diagnosis and treatment;the present situation and existing problem of traditional Chinese medicine and Western medicinein treatment of chronic heart failure;development trend of chronic disease management and the effects of Chinese medicine clinical pharmacists in the treatment of diseases.This part treats patients who were in hospital of integrated traditional Chinese and Western Medicine Hospital Department of cardiology as the research object,carries on the discussion to the Chinese medicine clinical pharmacists work mode,clinical pharmacists how to cut into the treatment of patients with chronic heart failure,how to assist doctors to formulate individualized medication.the curative effect of managing patients with chronic heart failure provides reference for clinical pharmacists work.The main contents and results of clinical Chinese pharmacy model are discussed:(1)based on the analysis of the cases of chronic heart failure in Chinese medicine combined with traditional Chinese medicine and Western Medicine(TCM)Department of Cardiology,the author summarized the medication law and the characteristics of the treatment plan for the treatment of chronic heart failure with integrated traditional Chinese and Western Medicine(TCM).(2)The clinical pharmacists participation of traditional Chinese medicine combined Western medicine pharmacy service in treatment of patients with chronic heart failure based on the management mode of pharmacy.The second part is about the practice of pharmacy.In the part of pharmaceutical practice,the author aims to provide knowledge storage for clinical pharmacists in clinical practice through the understanding and learning of basic pharmaceutical practice in Chinese and Japanese hospitals,so as to ensure better service for pharmacists in clinical practice.The author has a full understanding of traditional Chinese medicine pharmacy?drug control room?preparation room,and learn a lot of knowledge from the process of practice in middle school,to learn how to deal with the relationship with the patients,laying the groundwork for the clinical pharmacists work,by means of practicing in the hospital,In summary,through the discussion of clinical traditional Chinese pharmacology management mode and the study of clinical pharmacy practice,clinical pharmacists combine the knowledge to practice,to explore suitablemanagement mode for clinical medication,it can meet
