Osteoarthritis of the knee(Knee Osteoarthritis KOA)is one of the main diseases that seriously affect the quality of life of patients,and has become the focus of attention of scholars at home and abroad.Epidemiological investigation showed that the prevalence of disease was 95.6%,of which 60 of people over the age of high incidence of 49%[1],not only affects the quality of life of patients,but also of ten aggravates the economic burden of the patients.Needle knife therapy in the treatment of knee osteoarthritis clinical effect is significant,the researchers studied the mechanism of acupotomy in the treatment of KOA from morphology and molecular biology,KOA damage and repair of the soft tissue is relatively small,this experiment through the observation of apoptosis of rabbit model of the rectus femoris muscle cells,to investigate the effect of acupotomy treatment of osteoarthritis of knee arthritis,injury and repair,clear mechanism of acupotomy treatment KOA,treatment methods and theoretical basis for acupuncture treatment of KOA.ObjectiveClinical and experimental study of knee osteoarthritis early center that,effect of needle knife therapy in the treatment of osteoarthritis of the knee were.In this experiment,the left hind limb extension method in rabbits with knee osteoarthritis model of fixed brake,using needle knife treatment,and the treatment were compared with electro acupuncture,to observe effect of acupotomy treatment on the expression of knee osteoarthritis of rectus femoris muscle cell apoptosis and Caspase-3,to reveal the mechanism of acupotomy in the treatment of osteoarthritis of the knee may be from the rectus femoris the role of apoptosis,and provide a theoretical basis for the treatment of knee osteoarthritis acupotomy.MethodsIn June at the age of 24 healthy New Zealand rabbits(male and female),were randomly divided into four groups:normal group,model group,electro acupuncture group,acupuncture group,the left hind limb immobilization in extension position preparation of knee osteoarthritis model.The knee X-ray observation of rabbit model of KOA imaging changes by HE staining,light microscope to observe the morphological changes of the KOA model of rabbit rectus femoris muscle cells,TUNEL method was used to detect the rectus femoris positive apoptotic nuclei number,number of the immune positive expression was observed in the group of rectus femoris Caspase-3 method,to study acupuncture intervention on apoptosis of rabbit model of femoral cells in the medial rectus.Results1 Aftermodeling 6 weeks,the general situation of rabbit were observed,results showed that:the model of rabbit knee joint local swelling,spirit is dispirited,activity was significantly reduced(left lower limb in the extended state),toe hyperemia clear,obvious limp.The rabbit knee joint imaging X-ray observation group,normal rabbit knee joint sclerosis and cystic change was not obvious,femoral condyle,tibial margin of bone hyperplasia were not there,the knee joint edge no osteophyte emerged,uniform subchondral bone sclerosis and cystic change.Model group:af ter modeling,X-ray radiographic knee radiographs showed the knee joint structure changed obviously,visible left knee joint bone hyperplasia,tibial plateau appears uneven surf ace,the rabbit knee joint medial and lateral gap was narrowing,the knee joint was unstable state.The results show that this method not only effectively,and the effect is good.2 The HE of rectus femoris of rabbit staining results showed:af ter modeling,the model group increased muscle cells,nuclear transfer;muscle fiber coloring are dissolved degeneration,edema,wavy,segmental twist;Leydig cells and connective tissue proliferation and scar formation,inflammatory cells(including the number of neutrophils)infiltration;most Acupotomy intervention,the muscle fibers were smooth and no obvious distortion change that Acupotomy intervention can promote the benign transformation of rectus femoris muscle tissue,has a good effect on the restoration of the rectus femoris muscle.3 TUNEL results showed that:af ter modeling,the model group of rabbit rectus femoris TUNEL positive cell counts in normal group was significantly increased,there was significant difference between the two groups(P<0.01);treatment group compared with the normal group had no significant difference(P>0.05);treatment group was significantly lower than the model group,the two have significant differences(P<0.01);Electroacupuncture group than in the normal group were significantly different(P<0.05);there was significant difference between the acupuncture group and the model group(P<0.05);there was significant difference between the acupuncture group and the electroacupuncture group(P<0.05).4 The expression of Caspase-3 showed:after modeling,the model group increased significantly compared with normal group,there was significant difference between the two groups(P<0.01);treatment group had no significant difference with the normal group(P<0.05);treatment group was significantly lower than the model group,with significant differences between the two groups(P<0.01);electric acupuncture group compared with the control group no significant difference(P<0.05);Electroacupuncture group and model group was significant difference(P<0.01);there was no significant difference between acupuncture group and electroacupuncture group(P>0.05).Conclusion1 Acupotomy intervention can reduce the inflammatory reaction and good knee osteoarthritis rabbit tissue,promote tissue repair.2 Acupotomy intervention can reduce the number of KOA model of rabbit rectus femoris muscle cell apoptosis,reduce the pathological damage of knee osteoarthritis.3 Acupotomyintervened by down regulating the expression of Caspase-3,inhibit the KOA model of rabbit rectus femoris muscle cell apoptosis,which may be an important mechanism of acupotomy in the treatment of KOA.
