Objective:To observe the clinical efficacy of acupotomy based on the ultrasonic multi-modal technology in the treatment of knee osteoarthritis along with the focal point of meridional fascia, the new ideas and methods are provided for clinical research by improving the effectiveness of acupotomy treatment of knee osteoarthritis. Methods:From December 2019 to December 2020,92 cases of KOA patients with grade I to III who accorded with the diagnostic criteria and the research inclusion criteria of KOA were selected from the outpatient or inpatients of the Department of Orthopedics and Traumatology of the People’s Hospital Affiliated of Fujian University of Traditional Chinese Medicine.According to the random number table method,92 patients with KOA were proportionably and randomly divided into the trial group(ultrasound-guided acupotomy treatment group)and the control group(non-visual acupotomy treatment group),and 46 cases were in each group. The trial group was treated with ultrasound-guided acupotomy to relax the focal point of meridional fascia, and the control group was treated with non-visual acupotomy to relax the focal point of meridional fascia. The treatment points in both groups were “Hedingci”,“ Binneixia”,“Binwaixia”,“Yinlingshang”.The acupotomy treatment is performed once a week,4 times as a course of treatment, and a total of a course of treatment. The changes in observational indicators at three points in time(pre-treatment,post-treatment,3 months after treatment)were recorded, such as the WOMAC scores(pain score, joint stiffness score, daily activity score, and comprehensive score),the characteristics of the rectus femoris ultrasound morphology(pinnate angle, muscle thickness, muscle cross-sectional area, and elastic strain rate),and the thickness of articular cartilage. The Clinical efficacy evaluation was performed1 week after treatment.SPSS 23.0 software is used to organize research data and statistical analysis.Results:1.The WOMAC scores were used to assess the effects of both groups on knee function during pre-treatment,post-treatment,and 3 months after treatment. Both groups were able to reduce WOMAC scores, improve knee function, and the trial group was better than the control group(P<0.05).The effect of the trial group was best for pain relief during 3 months after treatment, while the score for joint stiffness and daily activity was the best on the post-treatment(P<0.05).2.The thickness of the femoral articular cartilage was used to evaluate the effects of cartilage degression on both groups during pre-treatment,post-treatment,and 3 months after treatment. The two groups had not yet shown that they could delay articular cartilage cataplasia, and the results of the two comparisons between the groups showed no difference between them(P<0.05).3.The ultrasound morphological characteristics were used to assess the effects of two groups on the rectus femoris pre-treatment,post-treatment,and 3 months after treatment. Both groups were able to improve the characteristics of the rectus femoris ultrasound morphology(P<0.05),and the trial group was better than the control group(P<0.05).The effect of the trial group improved the characteristics of the rectus femoris ultrasound morphology(pinnate angle, muscle thickness, muscle cross-sectional area, and elastic strain rate)at 3 months after treatment(P<0.05).4.The typical correlation analysis is used to analyze the correlation between the WOMAC score and the characteristics of the rectus femoris ultrasound morphology. There is a correlation between the WOMAC score and the characteristics of the rectus femoris ultrasound morphology(the typical correlation coefficient is 0.424,P<0.05).The degree of joint stiffness is the main reason that affects the function of the knee joint, and the cross-sectional area has a great influence on the function of the rectus femoris.5.The marked efficiency of the trial group was 97.82%,the marked efficiency of the control group was 84.78%,and the marked efficiency of the trial group was higher than that of the control group(P <0.05).Conclusions:The treatment of knee osteoarthritis with acupotomy based on the ultrasonic multi-modal technology along with the focal point of meridional fascia can regulate the morphology and hardness of rectus femoris muscle tissue, effectively relieve knee pain, improve joint function and improve daily behavior ability, and the efficacy is better than traditional acupotomology treatment.
