Clinical Study On The Changes Of TCM Syndromes Before And After Chemotherapy In Colorectal Cancer

This subject uses Retrospective study on method to study the colorectal cancer before and after chemotherapy in patients with postoperative adjuvant chemotherapy for the first time on the change law of syndrome of traditional Chinese medicine. The study discusses the TCM etiology and pathogenesis,Characteristics and effects of chemotherapy on colorectal cancer syndromes,to provide theoretical basis for clinical use and guide the treatment of traditional Chinese medicine in chemotherapy in patients with colorectal cancer.Results:Firstly, before chemotherapy the colorectal Cancer syndromes are mainly excessive syndromes,including phlegm syndrome,stagnant blood syndrome and dampness syndrome; after chemotherapy phlegm syndrome reduces,while stagnant blood syndrome and dampness syndrome increase,especially damp stagnancy syndrome.Deficiency syndromes also increase,especially qi deficiency and blood dificiency.Secondly, before chemotherapy the colorectal Cancer syndromes are mainly in the spleen, while the TCM syndromes mainly are in the spleen and stomach after chemotherapy.Thirdly,most tongue manifestations are dark tongue(including dark red tongue and light dark tongue),red tongue(including red tongue and dark red tongue), teeth printed tongue,white coating and greasy coating before chemotherapy,after chemotherapy red tongue significantly reduces,while pale tongue,light dark tongue, teeth printed tongue and fat tongue increase.White coating and greasy coating are still the main part.Forthly,most pulse conditions are deep pulse,slippery pulse and taut and slippery pulse before chemotherapy;after chemotherany single-pulses and taut and slippery pulse reduce,while string slow pulse and string minor vein increase.Not only that other conbined pulses appear, including deep and minor pulse and deep and weak pulse.Fifthly, before chemotherapy the TCM syndroms of colorectal cancer patients in differert TNM stage are mostly excessive syndromes,expecially phlegm syndrome,stagnant blood syndrome and dampness syndrome;after chemotherapy the excessive syndrome are almost as much as the deficiency syndromes.The deficiency syndromes significantly indreas,especially qi deficiency.But the excessive syndromes partly reduce, especially phlegm syndrome,while the damp stagnancy syndrome increases.Sixthly,before chemotherapy,the TCM syndromes of patients with adenocarcinoma are mostly qi deficiency,phlegm syndrome(including phlegm-heat syndrome and phlegm-turbidity syndrome), stagnant blood syndrome and dampness syndrome(indluding cole-dampness syndrome and damp stagnancy syndrome); while the TCM syndromes of patients with mucinous adenocarcinoma are mainly qi deficiency,stagnant blood syndrome and damp stagnancy syndrome.After chemotherapy, the TCM syndromes of patients with adenocarcinoma are still mostly qi deficiency,phlegm syndrome(including phlegm-heat syndrome and phlegm-turbidity syndrome), stagnant blood syndrome and dampness syndrome(indluding cole-dampness syndrome and damp stagnancy syndrome),and the blood deficiency syndrome increases; while the TCM syndromes of patients with mucinous adenocarcinoma are mainly qi deficiency,stagnant blood syndrome and damp stagnancy syndrome,and the yang deficiency syndrome increase. Seventhly, the qi deficiency syndrome of patients over60years old significantly increase after chemotherapy.Eighthly, Comparing patients whose chemotherapy program is FOLFOX and patients whose chemotherapy program is combination of Oxaliplatin,calcium folinate and tegafur,the changes of syndrome of have no obvious difference after chemotherapy.Conclusion:Firstly,the cause and pathogenesis of large intestine cancer are mainly dampness, phlegm and blood stasis.The TCM syndromes of large intestine cancer are complex and diverse, mostly performancing deficiency complicated with excess after chemotherapy.Secondly, the one hand chemotherapy can expell evils(damp-heat,phlegm and phlegm-heat),the other hand chemotherapy damages genuine qi(qi,blood and yang) while adding evils(blood stasis and dampness).Thirdly, before chemotherapy the treatment of traditional Chinese medicine is mainly warding off evils by clearing away heat,resolving phlegm,resolving dampness and removing blood stasis, supported by strengthening the genuine. Before and after chemotherapy we can strengthen the genuine supported by expelling evils.The methods for expell evils are replenishing qi and activating blood circulation and strengthening the spleen and harmonizing the stomach.The methods for expelling evils is resolving dampness,resolving phlegm and promoting blood circulation to remove blood stasis.
