| Threatened abortion(T.A.) is a common gynecology disease, considered closely related to the imbalance of the kidney, liver and spleen function, especially to the deficiency of the kidney. As a result, supplement of the kidney and preventing miscarriage are considered as the main treatment.'TAIUONGBUAN' is referred to threatened abortion(T. A.)in western medicine and it's treatment is mainly repairing progesterone and HCG based on the patient's different circumstances.This paper have carried out feedback analyses, patients' regular information, different symptoms, severity of the disease and the relationship between different disease classification and therapeutic effect on the Chinese western medicine treatment for the threatened abort, i onObjective:To investigate on the Chinese western medicine therapeutic effects on threatened abortion; analyze on ihe treatment influencing factors. To provide a certain clinical basis on the treatment of threatened abortion.Methods:Between the year2007,march-2011,march, the patients were selected from dongzhimen hospital gynecology department hospitalized patients, age ranges from20-40, qualified to the100cases of kidney deficient type threatened abortion, used western modieine(progesterone, ?-HCG) and Chinese medicine (mainly Shoutai Decoetion to carry out the treatment. The treatment groups were divided according to the Standard Integrated Evaluation Treatment Effects,80cases were classified as the recovered, effective and got better group,20cases were classified as ineffective group. Accumulate and organize patient's clinical data, and to analyze on the influencing factors to the treatment.Results1. Treatment Effect Analyses1.1Vaginal bleeding was majorly shown within the100cases when symptoms were clinically analyzed and is one of the main TA clinical symptoms. The next often shown symptom is lower back pain, lower abdominal pain, lower abdominal inflation feeling, constipation, thirsty throat, irritability etc. As a result, P value smaller than0.05corresponding to lower back pain, fatigue, pollakiuria and thirsty throat, is statistically significant. This shows that the therapeutic effect for lower back pain, fatigue and pollakiuria is effective; treatment result for thirsty throat is less effective.1.2From the Chinese Medicine disease classification analyses,100cases were all sorted as kidney deficiency type, but was also partially shown with other disease classification. Kidney deficient type was shown the most from the100cases, then is the kidney deficient with spleen deficient type, kidney deficient with blood heat type, kidney deficient with qi stasis type, and kidney deficient with qi blood deficient type. As a result, P value smaller than0.05(P<0.05) corresponding to kidney deficient type and kidney deficient with blood heat type is statistically significant. This shows the treatment for pure kidney deficient type threatened abortion is effective; treatment for kidney deficient with blood heat type is less effective.1.3According to the analyses of the state of illness and the treatment result of the100cases of threatened abortion patients, it is shown that the P value corresponding to the severe patients is less than0.05, which is statistically significant. This shows that the treatment for severe patients from the100cases of threatened abortion patients is less effective.2Relation between therapeutic effect and other factors2.1The mean age of the effective group is31.25, the mean age of the ineffective group is31. According to the logistic feedback analyzes, therapeutic effect is not influenced by the age of the patient.2.2According to the analyses of the frequency of previous natural abortion, the result shows that the P value, smaller than0.05, corresponding to the frequency of previous natural abortion3times, is statistically significant. This shows that the more frequency of previous natural abortion, the more therapeutic effect decreases. The therapeutic effect is not influence by the artificial abortion. 2.3From the analyses of pregnancy period, As a result, the P value smaller than0.05, corresponding to the pregnancy period less than8weeks and over10weeks, is significant. This shows that threatened abortion pregnancy period less than8weeks is effective; pregnancy period over10weeks is less effective.2.4Serum ?-IICG value t tests shows that serum ?-HCG value level of pregnancy period between8-10weeks and pregnancy over10weeks effective group is higher than the ineffective group. Effective group blood HCG value gradually increase to its climax on the10th week of pregnancy, maintaining its'level from then. Ineffective group blood HCG is lower than the effective group.2.5Serum P valueT test shows that the serum P value level of the effective group for pregnancy period over10weeks is higher than the ineffective group. The P value of the ineffective group for pregnancy period between8-10weeks starts to decrease, therefore pregnancy period between8-10weeks is the critical period to prevent miscarriage.conclusions1The relation between lower back pain, fatigue, pollakisuria and treatment result is positively interrelated, thirsty throat and treatment result is negatively interrelated. The relation between other symptoms and treatment result is not related.2The relation between pure kidnny deficient type and treatment result is positively interrelated. The relation between kidney deficient with blood heat type is negatively interrelated.3The relation between severe patients and therapeutic effect is negatively interrelated.4The treatment is not influenced by the patients' age.5Patient with previous natural abortion frequency over3times and the therapeutic effect is positively interrelated. The therapeutic; effect is not influenced by the frequency of artificial abortion.6The relation between pregnancy period less than8weeks and therapeutic effect is positively interrelated. The relation between pregnancy period over10weeks and therapeutic effect is negatively interrelated.7Effective group blood HCG value gradually increase to its climax on the 10th week of pregnancy, maintaining its'level from then. Blood HCG value of the ineffective group is distinctively lower than the effective group.8Serum P value level of the effective group for pregnancy period over10weeks is higher than the ineffective group. The P value of the ineffective group for pregnancy period between8-10weeks starts to decrease, therefore pregnancy period between8-10weeks is the critical period and must pay close attention when clinically preventing miscarriage. |
