Investigate The Care Service Needs And Life Quality Of The Chronic Eldly Under Different Pension Mode In NanChang

Objective:Facing the increasingly serious aging and chronic disease status,solving the problem of the elderly population of chronic disease is imminent.Chronic,longer duration,slow recovery,easy to produce complications,can cause disability and dysfunction,leading the rapid increase in the number of disabled elderly.Seriously affect their life quality.This study aims to formulate government policy about chronic diseases of the eldly pension services;providing the needs of the pension for the status reference data and material resources permit,and exploring the life quality influencing factors of the chronic elderly among different modes,in order to improve the life quality of chronic diseases of the elderly.Methods:1.This topic uses the literature research to study the topic related concept as well as the domestic and foreign scholars about the research progress of the chronic disease eldly service demand.2.From July to November 2014,using the method of qualitative research on the chronic elderly,30-60 chronic elderly were carried out semi-structured interviews about Pension literature demand for services.The interview data was collected,transcribed,analysised by Claizzi phenomenological analysis method.Design the "chronic care services for the chronic elderly needs questionnaire."3.From December 2014 to March 2015,11 experts was inquiried,to determine the content validity.In April 2015,drawn 50-60 chronic elderly to pre-experiment questionnaire internal consistency and test-retest reliability analysis.4.From May to December 2015,use "chronic care services for the chronic elderly needs questionnaire" and "SF-36 summary QOL," to test the basic situation of the extracted 424 chronic elderly,thier life quality,retirement wishes,and pension service needs.Results:1.The total content validity of the scale is 0.92,total test-retest reliability was 0.876.By index system based on the scale,Cronbach alpha coefficient is 0.91,daily life care service dimension Cronbach alpha coefficient is 0.94,the medical care service dimension Cronbach alpha coefficient is 0.72,the spiritual and cultural service dimension Cronbach alpha coefficient of 0.78.2.The life quality of the chronic elderly under different pension mode,on the whole,the chronic elderly in the community home care has the highest life quality,family pension,followed.The chronic elderly in the Pension institutions has the lowest life quality.Compared institutions mode with community home-based care mode,the difference was statistically significant(P <0.05);Compared institutions mode with family pension mode,the difference was statistically significant(P <0.05);Compared the community home-based care mode with family pension mode,the difference was not statistically significant(P> 0.05).3.ANOVA and multiple linear regression analysis showed that the life quality influence factors of the chronic elderly has slightly difference in different pension mode.But all are subjected to age,actual monthly income,the type of chronic illness species,the impact of daily living currently.The life quality of the chronic elderly whose pension mode is family pension mode or community home-based care mode are governed by education level,marital status influence,husband/wife’s physical condition,the degree of harmonious family relations,the current physical examination frequency.In addition,the life quality of the chronic elderly in family pension nmode is also affected by the number of years suffering from chronic diseases.4.If the daily living and health conditions weakening,the elderly population who choosing institutions mode will be increasing sharply;The chronic eldly from different pension mode have different views about advantages and disadvantages of the various pension mode,to a certain extent,affecting their choosing pension mode.;The chronic elderly in different modes has different pension chronic care service needs;Life quality and pension service needs of the elderly chronic has negative correlation.Conclusion:1.The questionnaire of chronic elderly pension service needs is a good scale,which has good reliability and validity.2.The quality of life and pension need of the eldiy with chronic diseases under diffrernt pension patterns are different.Services need to be targeted to meet the needs of chronic care services for the elderly and improve their quality of life.
