Objective: Based on the analysis of the application status of antimicrobial agents in a tertiary specialized hospital from 2013 to 2015,the study explores the rationality of the application of antimicrobial agents in clinical treatment from various angles and tries to explore a series of more scientific and normative method of drug application.This provides a good reference for improving the therapeutic effect of the disease and reducing the patient's drug expenditure.Approach: the study from the beginning of 2013 to the end of 2015 in the specialized hospital,studying the clinical treatment used antimicrobial drugs and all the income of the drugs;The use of antimicrobial drugs were arranged in the popular order in the past three years of the specialized hospitals,and 46985 related outpatient prescriptions were drawn to explore the components involved in antimicrobial prescriptions,the proportions of each of them,the effects of common use,unreasonable situation and so on;Randomly extracted non-surgical patients 540 cases from the hospital in January 2013-December 2015,to study the use of antimicrobial drugs in the process of treatment in patients who have not undergone surgical treatment,DDDs sequencing,ingredients of antimicrobial prescription,the proportions between them,and the use of unreasonable problems in prescribing;For the surgical application of surgical inpatients,it surveyed the application of antimicrobial agents in the perioperative period from1080 cases of class I incision surgery,the composition of antimicrobial drugs and the distribution and proportion between them.According to the relevant rules of relevant antimicrobial drug guidance documents to investigate the application of antibiotics in the above patients.Making a specific table for the above statistics,while the extent of the rationality of application of antibiotics in the specialized hospital was assessed.Results: The ratio of antimicrobials accounted for10.37%?9.53% and 9.42%of the total drug income from 2013 to 2015,showing a gradual decline trend.The prescriptions included antimicrobial agents were 17.48%,13.07% and 10.52% of the outpatient prescriptions between 2013 and 2015 respectively.The proportion of the antimicrob ial agents in the patients who did not undergo the operation was 38.15% of all patients,while the proportion of patients at the same timewho used a variety of antibacterial drugs accounted for 19.26%.For the surgical applications of surgical inpatients between 2013 and 2015,the proportion of antimicrobial agents applied to the surgery of class I incision was 29.17%,22.78% and 25.83%,respectively,showing a decreasing level.In the 262 cases of class I surgery,the ratio of first-generation cephalosporins was 60.69%,the ratio of second-generation cephalosporins was 28.62%.143 cases were treated with antibiotic in the perioperative for less than 24 hours,accounting for 54.58%,and 20 cases more than seven days,accounting for 7.63%.Conclusion: There are still many unreasonable problems in the use of antimicrobial drugs in the specialized hospitals.The main problems were that the selected antibacterial grade was too high,the dosage was too big,the period of medication was long time,and higher proportion in using combined drugs without indications.Suggestion: It should further strengthen the management of the use of antibacterial drugs,implement hierarchical control for this kind of drugs,focus on the drug pathogenic examination,evaluate the drug sensitivity test,and adopt scientific and rigorous training to improve the level of application of antibiotics.
