Screening For Pre-diabetes Group In Uygur、Han And Study The Effect Of Telnet Health Management

Objective: The purpose of this study was to investigate the prevalence of pre-diabetes in uygur 、 han groups with physical examination of a grade A class three hospital in xinjiang,to understand the distribution of risk factors for pre-diabetes in uygur、han and to compare their differences;with the help of Internet technology building intelligent health management platform,to study the feasibility and intervention effect of telnet health management mode in pre-diabetes groups.Methods: Using the cross-sectional survey method,the diabetes risk assessment,basic investigation,physical examination and laboratory examination was conducted for 2,052 people who participated in a healthy physical examination at a grade A class three hospital in xinjiang from January to August 2017,to determine the sglycometabolism status and risk factors of pre-diabetes in object of study,210 pre-diabetic patients who met the inclusion exclusion criteria were randomly divided into experimental group and control group,the control group using normal healthy education,the experimental group implementing the telnet health management mode on the basis of normal healthy education,comparing the biochemical indexes and the self-management ability of the two groups before and after intervention.Results:(1)The proportion of pre-diabetes in uygur people were 23.1%,the proportion of pre-diabetes in han were 19.9%,the proportion of impaired glucose tolerance(IGT)was the highest in the pre-diabetes population.(2)The proportion of men with pre-diabetes and type 2 diabetes(T2DM)was higher than that of women.(3)The pre-diabetes population was mainly distributed among people over 40 years old.(4)The proportion of IGT,impaired fasting glucose(IFG)increased with the increase of waist circumference,IGT/IFG population proportion did not increase with the growth of waist circumference.(5)The proportion of IGT,IFG/IGT increased with the increase of uricacid level,and the proportion of IFG population did not increased with the increase of uric acid level.(6)The BMI,waist and waist-to-hip ratio of pre-diabetes in uygur were higher than han,creatinine,uric acid and triglyceride were lower than han(P<0.05).(7)Non-conditional multivariate Logistic regression analysis showed: pre-diabetes in uygur,age,waist,hip circumference,waist-to-hip ratio,greasy diet,excessive consumption of staple food,excessive meat consumption,uric acid and cholesterol are independent risk factors for pre-diabetes(P<0.05);pre-diabetes in han,gender,age,systolic blood pressure,history of high blood pressure,excessive consumption of meat,and cholesterol were independent risk factors for pre-diabetes(P<0.05);the increase of exercise time is the protective factor of pre-diabetes in uygur and han(P<0.05).(8)After 3months of telnet health management for pre-diabetes groups,the experimental group was significantly higher than the control group in health concept,exercise,compliance management and social environment change(P<0.05).(9)After 6 months of telnet health management for pre-diabetes groups,patients in systolic blood pressure,postprandial blood glucose,health concepts,self-efficacy,diet,exercise,compliance management,social environmental change is significantly higher than the control group(P<0.05).Conclusion:(1)The prevalence of pre-diabetes was severe,and there were differences in ethnic,age and gender distribution,while IGT was more widely available in pre-diabetes patients.(2)The prevalence of pre-diabetes in uygur population is higher than that of han nationality.(3)In terms of age and gender,people over the age of 40 and men are the target group and key intervention targets for pre-diabetes screening.(4)Central obesity,blood pressure,blood lipids and uric acid in different degree affect people’s blood glucose level with pre-diabetes,compared with han,uygur pre-diabetes have waistline,hip circumference,waist hip ratio,excessive consumption of meat,excessive consumption of staple food independent risk factors.(5)Telnet health management model for pre-diabetes patients is beneficial to long-term control of blood pressure and blood glucose,and it can obviously improve the self-management effect of knowledge-psychology-behavior,which is beneficial to prevent diabetes.
