Object:As one of chronic non-communicable diseases,diabetes not only affects people's health seriously,but also becomes the world public health problem.Diabetes mellitus and its complications on the patients caused by serious harm and the need for life-long treatment,the patient's family and society to bring heavy economic burden.With the continuous advancement of the basic Public Health Service,the function of Community health service has gradually become sound and perfect,and the new reform plan has taken "the promotion of basic public health service" as one of the key tasks.The National Family Planning Commission(formerly the Ministry of Health)has promulgated the "National Basic Public Health Service Norms"(2009,2011,2016,soliciting comments),all on the management of Diabetes mellitus service object,content,process and work requirements and evaluation indicators,there are detailed and clear provisions.The population structure of Dushanzi area is unitary,the aged 60 and above is 15.6% of the total population,and has already entered the aging society.Through the research and analysis on the management of diabetes mellitus in Dushanzi area,this study explores the effectiveness and authenticity of management,and lays a foundation for better local diabetes management,also work for other chronic diseases management summed up experience.Methods:Since 2010,the Dushanzi area started to set up electronic version of health records for local residents,the inputting rate is more than 95%,from the Karamay urban and rural residents health file system to the January 1,2010-June 1,2017 management of 1769 patients with diabetes related information.For patients with diabetes health management,before management(first visit as intervention)and after management(after last follow-up as intervention)the level of glycemic control,BMI(Body mass index),lifestyle and other comparative analysis of management rate and blood glucose control rate,blood lipid,blood pressure and other cases of comparative analysis,using a single factor method for statistical analysis.Daily work process,follow-up records in accordance with the "National Basic Public Health Service Norms"(2011edition)requirements,through the system engineer to obtain the relevant data,using Excel to establish a database,measurement data with the mean ± standard deviation,the count data used ?2 test,T-Test statistical analysis.Results:1.After follow-up,health management,Duzishan Diabetes management rate of 47.7%,standardized management rate of 96.4%,blood glucose Successrate of 68.5%,diabetes management indicators have reached and exceeded the national Basic public Health Service norms assessment indicators.2.After the standardized system of diabetes management,the lifestyle and BMI of patients with diabetes have changed significantly,the number of smokers,the number of drinkers and the frequency of health exercise is statistically significant,diabetes mellitus patients before and after management diastolic blood pressure,cholesterol reduction are aiso statistically significant.The medication rate of diabetes mellitus was 97%,the highest frequency was acarbose glucose.Conclusion:1.The health management of diabetes mellitus in Dushanzi area accords with the requirements of national Basic public Health Service assessment.2.Through the positive and effective diabetes health management in our district,we can improve the main metabolic indexes of diabetes patients,delay the development of diabetes and its complications,and improve the quality of life of patients.3.Constantly strengthening and improving the health management of diabetes requires the joint efforts of government,health management departments,medical workers and residents.
