Objective:The purpose of this meta-analysis is to investigate the clinical effect of laparoscopic versus open surgical treatment of adult umbilical hernia or paraumbilical hernias?Methods:We Searched the literature published in PubMed,Embase,Cochrane Library,CNKI,CB,WANGFANG databases about the clinical effect of laparoscopic and open surgical repair of adult umbilical or paraumbilical hernia in the past 20 years,The quality of the literature was evaluated using the Newcastle Ottawa Quality Assessment Scale(NOS)and Cochrane Bias Risk Assessment Form.The target data was extracted from the literature by inclusion criteria and all statistical analysis was processed by Review Manager 5.3 softwarer.Results:We identified 12 studies,enrolling a total of 16688 patients for this meta analysis,to study the clinical effect of laparoscopic and open surgical repair of adult umbilical or paraumbilical hernias.Our analyses found that open repair adult umbilical or paraumbilical hernias was associated with a higher risk of wound infection(OR =3.00,95%CI:1.79-5.05,I2=19%),wound dehiscence(OR =6.53,95%CI:1.56-27.29,I2=0%),recurrence(OR =4.26,95%CI:2.54-7.15,I2=26%)and longer length of hospital stay(MD=35.68,95%CI: 17.68-53.68,I2=99%),shorter operative time(MD=-16.44,95%CI :-31.19--1.68,I2=100%)compared to laparoscopic repair adult umbilical or paraumbilical hernias.There was no diference sidnificant in the risk of seroma(OR=0.75,95%CI:0.20-2.84;I 2=81%)or haematoma(OR =3.80,95%CI:1.97-7.32,I2=70%).Conclusious:In the repair of adult umbilical hernia or paraumbilical hernia,the best available evidence suggests that laparoscopic repair of adult umbilical or paraumbilical herniasmay be associated with a lower risk of wound infection,wound dehiscence and recurrence rate ? shorter length of stay but longer operative time.There was no significant difference in the risk of seroma and haematoma in two groups.
