Objective: To investigate the expression of serum TNF-? in patients with liver injury caused by anti-tuberculosis drugs in Han and Uygur nationality,and to compare the genetic differences,as well as analyzing the correlation between the expression of TNF-?and the degree of liver injury and its clinical significance,in order to explore the clinical significance of cytokine TNF-? in the process of liver damage caused by anti-tuberculosis drugs,to explore the early diagnosis in treatment of anti-tuberculosis drugs for clinical liver damage provides new train of thought and theory basis.Methods: 235 cases with hepatic injury caused by anti-tuberculosis drugs in thoracic hospital of the Xinjiang uygur autonomous region,Xinjiang shihezi university school of medicine in the first affiliated hospital were selected as ATDLI group,among which 100 cases were Han,135 cases were Uygur,aged 18 to 65 years old.Uygur ighur screening liver damage of patients were the original inhabitants in Xinjiang,parents had no history of interracial marriage.100 patients without liver damage were selected as the control group,42 cases were han,while58 cases were uygur,aged 18 to 65 years old.This study approved by the medical ethics committee,and sign the informed consent.2 ml venous blood of ATDLI group and TB control group were extracted,to centrifugal at 4000 r/min for 5 min,isolated from the upper serum,at 20 ? under test for storage.Serum alanine aminotransferase(ALT),aspertate aminotransferase(AST),total bilirubin(TBIL)were test by automatic biochemical analyzer determination.The determination of serum TNF-? was test by ELISA.Using SPSS17.0 software to analyze data,measurement data were expressed with mean±standard deviation,comparison between groups used independent sample t test,multiple sets of mean comparison between used single factor analysis of variance,correlation analysis used Pearson correlation test,inspection standards by ?=0.05,P<0.05 was statistically significant.Results: 1)The patient's age,gender,weight,height ofgeneral data comparison between Han and Uighurs in ATDLI group,there were no statistically significant difference.2)The serum TNF-? levels of Han and Uighurs in ATDLI group were: 31.55±12.35 pg/ml,35.92±14.59 pg/ml.The serum TNF-? levels of Han and Uighurs in the control group patients were: 21.72±7.18 pg/ml,29.86±8.61 pg/ml.The serum TNF-? levels of Uighur patients were significantly higher than that of Han patients(P<0.05).It showed that the genetic differences between the two peoples.3)The levels of ALT,AST and TBIL in Uighur patients were higher than those of Han patients,but there was no statistically significant difference(P > 0.05).4)Through the Pearson correlation test and found that serum TNF-?expression and ALT,AST of ATDLI patients were positively correlated,r: 0.544,0.475.There was no correlation between TBIL and TNF-?(P>0.05).5)The serum TNF-? expression of patients with severe liver damage of Han and Uighurs in ATDLI group were higher than that in mild patients,statistical analysis shows that patients with severe liver damage of Han compared with mild group as a result,P=0.015;patients with severe liver damage of Uighurs compared with mild group show that P < 0.05.There was statistically significant between severe and mild liver damage(P<0.05),and there was no significant difference between the moderate group and mild group,severe group(P>0.05),patients of two national in ATDLI group,serum TNF-? expression quantity can increase with the increase of the liver damage degree.Conclusion: 1)TNF-? was involved in the pathogenesis of liver damage caused anti-TB drugs.It involved in the occurrence and development of inflammation and liver inflammation changes,and it can be observed as the development of the detection index.2)The change of TNF-? inside the Uighurs and the Han people had their characteristics,the ethnic differences may be related to genetic polymorphisms,diet and environmental factors of the two groups.3)TNF-? not only can be used as monitoring indexes of early diagnosis of ATDLI,but also it can be used as a judgment of disease severity and prognosis in patients with important indicators.
