Investigate The Relationship Between Metabolism Indices And Thyroid Function In Patients With T2DM

BackgroundDiabetes mellitus is a kind of the body chronic metabolic diseases,the main feature is chronic hyperglycemia.The prevalence of diabetes in epidemiology was 11.6%.Type 2 diabetes was found to have the highest prevalence among them.Type 2 diabetes mellitus can performance for metabolic disorders and polyhormone abnormalities,the latter including thyriod hormone.Thyroid dysfunction include(subclinical)hypothyroidism and hyperthyroidism.Past studies shows: thyriod hormone may be involved in the occurrence and development of T2 DM,T2DM often accompanied by abnormal thyroid function.However,abnormal thyroid function is closely related to T2 DM and impactthe developm-ent and evolution of the diease? ObjectiveRetrospective analysis the change of metabolism indices in patients with T2 DM.Investigate the relationship between thyroid hormone and metabolism indices.To understand and assessment the clinical value of the patients with T2 DM.MethodsSelect detailed information on 420 cases of hospitalized patients with T2 DM in endocrinology department(admission time between October 2016 and October 2017),all of the patients complete check and collect datas within 48 hours.Including sex?age?diseasehistory?body mass index,waist circumference,WHR,and test result of total cholesterol?triglyceride?low density lipoprotein-cholesterol?HbAlc?fasting blood glucose?fasting C peptide?blood urea,BUN?serum creatinine,Scr?blood uric acid,UA?24h urinary microprotein,QT24UMP?thyroid hormone level,FT3?FT4?TSH and A-TPO?Measure arterial blood vessels ultrasonography.Stratified statistics according to the results of the thyroid hormone level,analysis of the differents in various indicators on the patients with T2 DM.Determine whether there is a statistical difference(P<0.05),then draw a conclusion.Results1.After inclusion and exclusion criteria,a total of 420 cases were collected.There were280 males,140 females.28 patients with hypothyroidism,66 patients with subclinical hypothyroidism,4 patients with subclinical hyperthyroidism,1 patients with hyperthyroidism,321 patients with thyroid function is normal.2.The differences of FBG?HbA1c?TC and renal function betweenhypothyroidism and thyroid hormone normal group were statistically significant(P<0.05).3.According to the different of HbA1 C divide into 3 groups,with theriseof HbA1 C level,FT3 rise,TSH decline,they were statistically significant(P<0.05),FT4 have no statistical significance(P>0.05).4.According to the different of course of the disease,divide into 3groups,with the rise of time(years),the prevalent rate of subclinical hypothyroidism rise.The prevalent rate of Hyperthyroidism,subclinical hyperthyroidism and hypothyroidism have no statistical significance(P>0.05).5.According to the different of BMI level divide into 3 groups,with therise of BMI,TSH level rise,they were statistically significant(P<0.05).FT3 ? FT4 have no statistical significance(P>0.05).6.According to the different of WHR divide into 3groups,with the riseOfWHR,FT3 decline,TSH rise,they were statistically significant(P<0.05).Variration ofFT4 have no statistical significance(P>0.05).7.According to the different of 24 h urinary microprotein divide into 3 groups,with the rise of 24 h urinary microprotein level,FT3 decline,TSH rise,they were statistically significant(P<0.05).FT4 have no statistical significance(P>0.05).8.According to the different of thyroid hormone level divide into 4 groups,with the rise of FT3,HbA1 C level rise,TC,LDL-C,Scr level deline,they were statistically significant(P<0.05).ConclusionThere are high proportion of thyroid disfunction in the pations of T2 DM.There are some correlations between thyroid hormone level and metabolism indices in pations with T2 DM.Patients with thyroid dysfunction is high occupancy in T2 DM,we need to pay more attention on it.It has certain clinical significance in terms of assessment severity with the disease of T2 DM.
