| BackgroundThe structure of the tendons and ligaments at the point of attachment to the bone is called the terminal area,and the damage in this area can be caused by many factors,and the terminal degenerative disease caused by trauma strain is called the terminal disease?Enthesiopathy?[1].It mainly includes Achilles tendinitis,plantar fasciitis,humeral epicondylitis and so on.Among them,Achilles tendinitis is the most common anddanger-ous.With the rapid development of the national fitness campaign,the incidence of these diseases has increased significantly,which has become a hot issue to be solved in sports medicine.This study retrospectively analyzed inpatients and outpatients in January 2015-January 2017 treatment of the clinical data of 69 patients with early Achilles tendinitis in the Chinese people's liberation army 150 central hospital,to study the efficacy difference of radiofrequency ablation surgery and conservative treatment,and to evaluate the necessity of a plaster cast after radiofrequency ablation,aims to provide theoretical support for the clinical treatment of Achilles tendinitis.PurposeThis subject adopts digital pain rating scale?NRS?score and the ankle surgery society of America?AOFAS?score to do the follow-up evaluation about the 69 patients with early Achilles tendinitis the curative effect of different treatments for 1 month,3 months after treatment,12 month follow-up evaluation,to explore the therapeutic value of radiofrequency ablation therapy early Achilles tendinitis.MethodWe have retrospectively analyzed the clinical data of 69 patients with early Achilles tendinitis treated in our hospital from January 2015 to January 2017,among them were 51soldiers,12 sports fans and 6 others.The age range is 16 to 40 years old,with an average age of 27.8 years old.There were 37 cases on the left,32 on the right and 0 in bilateral.The69 patients were randomly divided into surgery group?plasma radiofrequency ablation?and control group?traditional treatment group?,The operation group had 33 cases?27males and 6 females?,and 36 cases in the control group?27 males and 9 females?.There was no statistically significant difference between the operation group and the control group on the time of treatment,height,weight,age and pre-treatment pain.The two groups are comparable.The control group treatment:?1?Ankle brake?Plantar flexion 1 0°15°?,the braking time for eight weeks.?2?The capsules were taken orally twice a day,200mg,and changed to200mg once a day after one mouth,then take 3mouths.?3?Infrared high temperature treatment once a day,15 days as a course of treatment,3 courses,interval 15 days.The operation groups were treated with radiofrequency ablation,and the operation groups were randomly divided into surgery group 1?brake?and operation group 2?non-braking?.Postoperative brake group:Ankle brake?plantar flexion 1 0°15°?,the braking time for eight weeks.Postoperative activity group:There is no fixed with plaster after the surgery,and the bed rested for three days.Then we use digital pain rating scale?NRS?on two groups of patients with postoperative 1 month,3 months,12 months follow-up,and make curative effect analysis.In addition,the pain digital evaluation scale?NRS?score and the American academy of ankle surgery?AOFAS?were used to observe the therapeutic effect of the surgery group 1 and the Postoperative activity group after 1month,3 months,12 months.SPSS19.0 software was used for statistical analysis.ResultAll the patients were followed up 1 month,3 months,12 months after treatment.Surgical group compared with control group after treatment about the NRS pain index:The score of NRS pain index was 3.55±1.18,1.88±1.02,1.27±0.84.in 1 month,3months,12 months.In the control group,the score of NRS pain index was 4.50±1.25,2.69±1.01,1.83±0.56in 1 month,3 months,12 months respectively.The P value of the pain score t test in each follow-up period was less than 0.05,indicating a statistical difference between the surgery group and the control group.This showed that the analgesic effect of the operation group was better than that of the control group.The functional score of AOFAS ankle joint was 69.48±3.22,76.52±2.85,81.94±1.84.in 1 month,3 months,12 months.In the control group,the functional score of AOFAS ankle joint was 60.22±2.60,67.44±3.26,72.72±1.78.in 1 month,3 months,12 months respectively.In each follow-up period,the p-value of t-test of ankle joint function was less than 0.05,indicating a statistical difference between the operation group and the control group,and this showed that the treatment effect of the operation group was better than that of the control group.In each follow-up period,the American society for ankle surgery?AOFAS?criteria for Postoperative brake groupand 2 for 1 month,3 months,12 months after operation,the results also show that the follow-up period function score t test results P values were more than 0.05,there has no statistical significance between the two groups.Conclusion1.Radiofrequency ablation surgical treatment effect is better in pain relief,improving exercise capacity,and promoting rapid recovery,it can also help to reconstruct the structure and function,and it should be the preferred treatment for early Achilles tendinitis.2.After radiofrequency ablation,the brake has no effect on the treatment of patients and the ankle function. |
