Objective:The research adopts controlled study design to discuss clinical curative effection of time-based scrapping for patients with lumbar disc diagnosed as TCM pattern of qi and blood stasis by conventional scrapping as controlled randomized group.to evaluate the patients' pain relief,symptoms improvements and the long-term effections,we analyze and compare with conventional scrapping,which can provide reference and reflection on the clinical application of the theory of Midnight-noon Ebb-flow.Methods:We selected patients who were eligible for inclusion exclusion criteria and from Orthopedic clinic of Suzhou hospital of traditional Chinese medicine during December 2016 and May 2017 according to the order of treatment.They were divided into two groups,contrast group(conventional scrapping),treatment group(time-based scrapping),40 cases for each group,The treatment group received midnight-noon ebb-flow of guasha(15:00-17:00),the control used other time of guasha(any time except 15:00-17:00),we used reducing method(strong power,quick speed)scrap low back du meridian(da-zhui acupuncture point to Changqiang point),bladder meridian(shen shu,qi hai yu,ta chang shu,cheng fu,Yinmen,Weizhong,Chengshan acupuncture points)and gallbladder meridian(Huantiao,Fengshi,Yanglingquan acupuncture points).each group was treated every 5 days,3 times for a period,2 periods treatment for a total Japanese Orthopaedic Association Scores(JOA)and McGill pain score(MPQ)were used to evaluate lumbar function and pain feeling before and after intervention.At the same time,Visual Analog pain Score(VAS)was used to measure subjective degree of pain before each intervention,the total score period of JOA was used to determine the efficacy,the recurrence of two groups' patients after 1 month and 3 months was observed after intervention,then we compare the different efficacy and long-term effection of the two scrapping methods.SPSS 18.0 software was used to analyse data.Results:(1)Baseline data analysis:Before the intervention,two groups of age,gender,distribution of lumbar disc,duration,state of illness,JOA,McGill pain score,VAS showed no significant difference(P>0.05).(2)After the intervention,JOA score of the treatment group increased to 24.63 ± 1.904 points and the control group increased to 22.35 ± 2.537 points..The difference between the two groups was statistically significant(P=0.047).(3)After the intervention,the McGill score of the treatment group decreased to 1.2 ± 0.87 points and the control group decreased to 5.0 ± 3.994 points.The difference between the two groups was statistically significant(P=0.005).(4)before each intervention,the VAS scores of both groups were lower than last time and the third comparison showed significant difference(P<0.01).(5)One month after the intervention,the effection showed no statistical difference between two groups and there was also no statistical difference at the end of three months between two groups(P>0.05).Conclusion:(1)Time-based scrapping and conventional scrapping can both enhance lumbar function in patients with LDH diagnosed as TCM pattern of qi and blood stasis,relieve patients' pain and improve the patient's symptoms and signs,but the effection of time-based scrapping is better,it can more improve the clinical curative effection,at the same time,long-term outcomes of time-based scrapping may be better.(2)The effection of time-based scrapping and conventional scrapping is accurate and safe,and they are worthy of clinical application.
