Research backgroud and objectiveChronic obstructive pulmonary disease referred to as COPD,due to long-term chronic inflammation caused by the repeated increase in the condition,the duration of the disease is difficult to cure,long-term drug maintenance treatment,resulting in a heavy economic burden on the patient,affecting the patient's mood,resulting in patients easily complicated with anxiety,depression and other emotional disorders,Seriously affect the quality of life of patients.With the modern biological-psychological-medical model proposed,the idea of“people-oriented”is even more prominent.When we treat diseases,we must not only use drugs to control the symptoms of the disease,but also need to pay more attention to the improvement of patients' quality of life.Traditional chinese medicine has always advocated the idea of holistic treatment.In the treatment of diseases,it not only controls the main symptoms,but also takes into account other aspects,so that multi-target,multi-site treatment of diseases.In this study,we used the method of peitu shenjing to treat chronic obstructive pulmonary disease with anxiety and depression,by observing the symptoms of TCM?the Hamilton Anxiety Scale(HAMA)? Hamilton Depression Scale(HAMD)?6-minute walking distance(6MWD),and slow Obstructive lung assessment test(CAT),improved version of dyspnea score(mMRC),lung function,and other indicators,explore the method of earth-improving gold improve the symptoms of COPD patients with anxiety,depression and the quality of life of patients.Methods60 patients with COPD combined with anxiety and depression,they belong to TCM syndromes types of feipi liangxu and xinshen shiyang,were selected from department of traditional chinese medicine and department of respiration clinic of Wuhan central hospital from October 2016 to March 2017.They were randomly divided into the treatment group and the observation group and 30 cases in each group.The observation group was given oxygen,bronchodilators,phlegm and other conventional treatment,and the treatment group was given peitu shengjin method treatment(self-made prescription drug by my tutor)based on the treatment of the observation group.Chinese herbal medicine were provided by Hubei Tianji Chinese Herbal Medicine Co.Ltd,once per day,after meals in the morning and evening,about 200 ml each time.Two groups patients were treated with continuous treatment for two months.TCM syndrome score,CAT?6MWD?pulmonary function?mMRC? HAMA and HAMD scores were evaluated before and after treatment.the data were analyzed by SPSS22.0 statistical software.Result1.In terms of clinical symptoms,after 2 months of treatment,both groups of patients in cough,sputum,chest tightness,shortness of breath and other symptoms were significantly improved than before treatment,compared with before treatment P <0.01,the difference was statistically significant However,there was statistically significant difference between the two groups,P>0.05.However,in the symptoms of fatigue,spontaneous perspiration,irritability,cold,upsets,and insomnia,the scores in the treatment group decreased,compared with P<0.05 before treatment.Statistically significant,but no significant change in the observation group scores,compared with before treatment P> 0.05,the difference was not statistically significant,between the two groups P <0.05,the difference was statistically significant.2.In terms of the efficacy of TCM syndromes,the TCM syndrome scores of the two groups after treatment were 29.10±10.27 and 44.90±10.13,respectively,which were lower than before treatment.Compared with P<0.05,the difference was statistically significant.P<0.01,there was a statistically significant difference.In the total efficacy of TCM syndromes,the total effective rate was 80% in the treatment group and 26.7% in the observation group.The difference between the two groups was P<0.05,and the difference was statistically significant.3.In terms of pulmonary function,the percentage of predicted volume of forced breathing(FEV1%)increased in the first seconds in both groups,and the ratio of forced respiratory volume to forced vital capacity(FEV1/FVC)in the 1st second also increased to some extent.Before comparison,the lung function of the two groups was improved to a certain extent(P<0.05).The increase of FEV1% and FEV1/FVC in the treatment group was significantly higher than that in the observation group.The difference between the two groups was P<0.05,and the difference was statistically significant.4.In terms of anxiety and depression,HAMA and HMAD scores of patients in the treatment group decreased significantly compared with that before treatment(P < 0.01),while HAMA and HAMD scores in observation group had no statistical significance compared with that before treatment(P>0.05),anxiety and depression did not change significantly.after treatment,the difference between the two groups was P<0.05,and the symptoms in the treatment group were significantly better than those in the observation group.5.In terms of quality of life,after 2 months of treatment,the mMRC,CAT,and m MRC scores in the treatment group decreased significantly,and the walking distance at 6 minutes was significantly longer,compared with P<0.01 before treatment,and the difference was statistically significant.However,there was no significant change in the 6-minute walking distance,CAT and mMRC in the observation group,and there was no significant difference between the observation group and the pretreatment group P>0.05.After treatment,CAT,mMRC,6MWD between the two groups P <0.05,the difference was statistically significant.ConclusionThe use of traditional chinese medicine decoction on the basis of routine western medicine,not only can significantly improve the symptoms such as cough,expectoration,shortness of breath,sweating,fatigue,but also can improve the symptoms such as insomnia,palpitations,anxiety,restless mood and so on,and can prolong six minutes walking distance,reduce CAT score and the degree of respiratory distress,improve the endurance of the patients and improve the quality of life of the patients.
