Clinical Effect Of Upper Limb Fumigation Combined With Open Reduction For The Treatment Of The Terrible Triad Of The Elbow

Objective:To observe the clinical effect of upper limb fumigation combined with open reduction in the treatment of terrible triad of the elbow.Methods:A total of 30 patients with standard terrible triad of the elbow were collected,Randomly divided into 15 cases of control group and 15 cases in test group.All cases were treated with the lateral elbow joint Kocher approach combined with the medial elbow joint approach,Functional rehabilitation training after operation.The control group was trained after the routine operation,On the basis of the routine postoperative rehabilitation training,the experimental group was treated with the upper limb fumigation,Observe the second weeks,1 months,2 months,3 months and 6 months after operation,X-ray of the elbow,the elbow flexion and extension and rotation activity,VAS pain score,Mayo elbow performance score,excellent and good rate,postoperative complications,SPSS 20 statistical software was used to carry out statistical analysis of the collected data.Result:1.All the 30 cases collected were followed up for 6 months.There were no significant differences in sex,age,cause of injury,distribution of injury side,and fracture classification.2.X-ray of the elbow:Fracture clinical healing time:all cases received clinical healing of fracture,the healing time of 10-18 weeks,the control group was 15.53±1.85 weeks,the experimental group was 13.67±2.06 weeks,compared the two groups after treatment,the healing time of fracture in experimental group was significantly shorter than that in control group,and the difference was statistically significant.(P<0.05).3.The elbow flexion and extension and rotation activity:comparison between second weeks after operation and 6 months after operation and two groups,there is no statistical significance in the elbow flexion and extension and rotation activity(P>0.05);The comparison between 1 months after operation,2 months after operation and two groups after 3 months after operation,there is statistical significance in the elbow flexion and extension and rotation activity(P<0.05).4.VAS pain score:comparison between second weeks after operation and 6 months after operation and two groups,there is no statistical significance in vas pain score(P>0.05);The comparison between 1 months after operation,2 months after operation and two groups after 3 months after operation,there is statistical significance in vas pain score(P<0.05).5.Mayo elbow performance score,excellent and good rate:sixth months after operation,Mayo elbow performance score was compared:the control group:85(70,90)points;the experimental group:95(85,100)points;6 months after operation,the mayo elbow performance score in the experimental group was significantly higher than that in the control group,the difference was statistically significant.(P<0.05).Sixth months after operation,the excellent and good rates of the two groups were compared:the experimental group:9 cases were excellent,4 cases were good,2 cases were medium,the excellent and good rate was 86.67%,the control group:7 cases were excellent,4 cases were good,4 cases were medium,the excellent and good rate was 73.33%.There was no statistical difference between the two groups after sixth months of operation(P>0.05).6.Postoperative complications:the control group:1 cases of heterotopic ossification,3 cases of joint stiffness;the experimental group:1 cases of joint stiffness,and two groups of patients had no statistically significant postoperative complications(P>0.05).Conclusion:After operation of terrible triad of the elbow,regular postoperative rehabilitation exercise combined with upper limb fumigation is better than routine rehabilitation training,It can promote the healing,reduce the pain,improve the function of the elbow joint,and effectively shorten the process of postoperative rehabilitation.
