Clinical Observation And Research On The Treatment Of Diabetic Foot With Sanshensihuang Jiangtang Capsule

Objective: Analyzing the mechanism of action about Sanshensihuangjiangtang Capsule on early Diabetic Foot by clinical observation and Clinical research,to provide more theoretical basis for future research and clinical promotion.Method: Clinical observation:The clinically admitted 90 patients with early diabetic foot(Wagner grading 0)were randomly divided into a control group and a treatment group with 45 patients in each group.The control group was given basic treatment and cilostazol tablets were added;the treatment group was given basic treatment and the Chinese medicine Sanshensihuangjiangtang Capsule was added.Treatments are 12 weeks.Record and analyze the TCM syndrome scores and clinical efficacy of each group before and after treatment,and test their FPG,2hPG,HbA1 C levels.Clinical research:On clinical observation day 0,7,14 th,21st,and 28 th day,blood samples were collected from clinical observation test patients at the time of morning venous blood,5ml each time,centrifuged at 3000r/min for 20 min,serum was separated and placed.Store in a refrigerator at-80°C.The levels of CRP,TNF-a,IL-6,AGEs,VEGF and IGF-1 in each serum were measured by enzyme-linked immunosorbent assay.And analyze the correlation between each group’s data and TCM syndrome scores.Result: 1.Clinical observation results:After being treated with Sanshensihuangjiangtang Capsule for 12 weeks,the total effective rate was 86.4% in the treatment group and 63.8% in the control group,and the curative effect in the treatment group was higher than that in the control group(P<0.05).In terms of TCM syndrome scores,the comparison between groups was significantly lower in the treatment group than in the control group(p<0.05);the intragroup comparison was significantly lower than before the treatment(p<0.05).The shallow sensation of the limb: Compared with the control group,the treatment group was significantly lower(p<0.05);within the group,the treatment group was significantly lower than before the treatment(p<0.05).In the lower limb arteriovenous ultrasound,ABI: between the groups,the treatment group was significantly lower than the control group(p<0.05);within the group,the treatment group was significantly lower than before treatment(p<0.05).The levels of FPG,2h PG,HbA1 c,AGEs,VEGF,and IGF-1 were significantly lower in the treatment group than in the control group(p<0.05).Within the group,the treatment group was significantly lower than before the treatment(p<0.05).2.Clinical research results: Inflammatory factors:On the 7th,14 th,21st,and 28 th day,the levels of hs-CRP,TNF-α,and IL-6 in the treatment group decreased compared with the previous detection time point(P<0.05),and there was no significant change in the control group.Growth factors:On the 7th,14 th,21st,and 28 th day,the levels of VEGF and IGF-1 in the treatment group were higher than those in the previous detection time point(P<0.05).There was no significant change in the control group.TCM syndrome scores:On the 7th,14 th,21st,and 28 th days,the scores of TCM symptom scores in the control group and the treatment group decreased compared with the previous detection time point.Compared with the control group,the treatment group decreased more significantly(P<0.05).AGEs level:On the 7th,14 th,21st,and 28 th day,the AGEs in the treatment group decreased compared with the previous detection time point(P<0.05).There was no significant change in the control group.Conclusion: Sanshensihuangjiangtang Capsule can treat the early Diabetic Foot.The mechanism may be to regulate blood glucose and blood lipids,increase the blood flow of the affected limbs,reduce the level of AGEs,and Adjust the levels of inflammatory factors and growth factors,so as to repair damaged blood vessels and nerves and play a role in the prevention and treatment of diabetic foot.
