Observation Of The Clinical Efficacy Of Detoxification Through The Network And Kidneys, And Early Treatment Of Diabetic Foot

AIM: To observe the curative effect of detoxifying and dia-collateralsBaoshen methods on the diabetic foot compare with 654-2 and study itsmechanism on the disorder of microcirculation.Methods: Patients with diabetic foot gangrene lesions who visited toYanbian University Hospital and Changchun University of TCM from October,2004 to February, 2007 and eliminate patients who complicating with todouble heart and renal inadequacy, myocardial infarction, nephros dutydepletion, asthma, pulmonary infection, advanced stage malignant tumor, et aland DF above grade 3 to deliver surgery, which were divided into treatmentgroup (n=32) and control group (n=27) by random sampling. Two groups werenot significantly different which in gender, age, course of DM, course ofDF, FBG on admission, et al(P＞0.05). Hongshen Chongcao Capsule (redginseng and Chinese caterpilar fungus mixed with invariably proportion and putinto capsule, offered by the Manufacturing Laboratory of Yanbian UniversityHospital, every capsule contains 0.15g) combine with detoxifying anddia-collaterals Baoshen Tang (Herba Siegesbeckiae 15g,Coptis chinensis5g,astragali 20g, forsythia suspensa 15g, anemarrhenae 15g, lycii radicis15g, salvia miltiorrhiza 15g, ramulus cinnamomi 12g, honeysuckle flower20g, all these were put into automatic decocting medicine machine by waterand saved 400 mL decoction per agent), 200 mL in the morning and eveningrespectively and have Hongshen Chongcao Capsule at the same time, totally1 months was a course of treatment. Under the same basic treatment, thetreatment group was treated with Chinese Materia Medicine, while thecontrol group with the rest 27 cases was treated with anisodamine(654-2). Themethods had been applied to the two groups differently for a course oftreatment. We observed the changes of blood rheology and nail foldmicrocirculation and etc in 59 patients with diabetic foot gangrene lesionsbefore and after treatment, who had been treated by reducing blood sugar andresisting infection.Results: Clinical observation on 59 cases showed good results: the total effects were 90.6% and 88.9% in treatment group and control grouprespectively. And there was an obvious difference between them (p＜0.05). Inthe treatment group, 4cases were cured, 18 cases showed improved and 3 caseineffective with the total effective rate of 68.8%. For the control group, 3 casewere cured, 13 cases showed improved and 3 cases ineffective, the totaleffective rate being 59.2%. The results indicated that the blood theology hadbeen significantly improved and the disorder of microcirculation had beenrelieved markedly after treatment in both groups. But difference betweentreatment group and control group were obvious, the comparison proved thecurative effect in the treatment group was better than the controlgroup(P＜0.05). So we can draw a conclusion that In the treatment group thetherapeutic result was better than that of control group.Conclusion: detoxifying and dia-collaterals Baoshen methods isfunctional for the treatment of diabetic foot gangrene and also for theimprovement of the disorder of microcirculation, witch provides an effectiveand safe method to treat diabetic foot gangrene with fewer side effects.
