Objective:To reveal the mechanism of "Zhaoshi tuoju tang"on treating diabetic foot gangrene(Traditional Chinese Medicine(TCM) syndrome of obstruction of blood circulation and toxic heat), based on the level of AGEs tested by modern science and technology, under the theory of syndrome differentiation in TCM. We hope to rich the TCM theory, excavate and save the TCM heritage, inherit and carrie forward the academic thought of Prof.Zhao,and disseminate his treating experience, which will benefit more patients with diabetic foot.Methods:Patients with diabetic foot gangrene lesions were randomly divided into two groups: Experimental group (the combination therapy group) containing 12 cases, and the control group (the Western Medicines therapy group) containing 14 cases. Under the same basic treatment, Experimental group was treated with "Zhaoshi Tuoju Tang" per os, and Ethacridine,while the control group was treated with Pentoxifylline and Ethacridine, with a course of 4-6 weeks. We observed the changes of glucose, blood lipid, white blood cell count,MCP,AGEs and etc in 26 patients with diabetic foot gangrene lesions before and after treatment, who had been treated by reducing blood sugar and resisting infection. Using the SPSS 13.0 statistics software, we indicated the determination data by the mean value±standard deviation ((?)±S),compared the curative effect in three groups by the rank-sum test, and the general condition by the T-test.Results:Compared the the clinical efficacy of the two group, the changes of glucose in the experimental group is superior to the control group before and after the treatment. After the therapy, serum lipids improved obviously and the level of the inflammatory factors decreased in different degree in both experimental group and the control group,but there is no remarkable difference between the two groups. AGEs in two groups decreased in a large degree,and the experimental group is superior to the control group.Conclusion:1.There is obvious curative effect in "Zhaoshi tuoju tang"on treating diabetic foot gangrene, which enhances the cure rate and efficient in enormous degree, reducing disability.2. "Zhaoshi tuoju Tang " can regulate the dyslipidemic-diabetic, reduce the AGEs level obviously, protect theβ-cell function, reduce the incidence of diabetic foot.3. "Zhaoshi tuoju Tang " cure the diabetic foot by ameliorating the MCP-llevel,and relieving inflammation,and to create the diabetic foot.Prospect:Diabetic foot is a general, chronic and interdiscipline difficult disease caused by several factors, because of its complicated mechanism. There is no specific diagnostic criteria, unitary differentiation of syndromes criteria and the clinical curative effect evaluation criteria of diabetic foot in TCM. In the study, there is some difficulties in collecting the patients who conform the differentiation of syndromes criteria. So we lack large sample, polycentric, polycentric control study and the depth of the study, which is hard to reveal the mechanism of "Zhaoshi tuoju tang"on treating diabetic foot gangrene. In the further study in the mechanism of "Zhaoshi tuoju tang",we could discuss the relationship betewee regulating inflammation by microRNA and microcirculation disturbance.
