| purpose:Observe the clinical curative effect before and after 4 weeks treatment of traditional Chinese medicine combined with laser treatment group(herein after referred to as treatment group)and simple laser control group(herein after referred to as control group)in the treat-ment of Qi-yin deficiency acquire stasis type of diabetic retinopathy,and compare the treat-ment group and control group before and after treatment.The changes of insulin-like growth factors-1(IGF-1)levels in serum and normal controls further explored the pathogenesis of diabetic retinopathy(DR)and the mechanism of combination therapy,in order to improve the effectiveness of DR treatment.method:According to the diagnostic criteria,inclusion criteria,and exclusion criteria of Qi-yin-deficiency acquire stasis type of diabetic retinopathy,screening and collection of Qi-yin defi-ciency acquire stasis type of diabetes retinopathy in Fujian University of Traditional Chinese Medicine Affiliated People's Hospital from January 2017 to December 2017,a total of 99 eyes of 60 patients(36 males and 24 females,youngest 35 years old and oldest 70 years old).Randomly divided into treatment group(Chinese medicine combined with laser treatment,5 1 eyes in 30 patients),control group(single laser treatment,48 eyes in 30 patients),simple laser control group was treated with laser alone;treatment group was treated with laser at the same time oral BuYangHuanwu decoction.30 normal healthy people in the physical examination center of Fujian University of Traditional Chinese Medicine Affiliated People's Hospital were collected as normal control group.Observe the BCVA,fundus photography,FFA and OCT in the treatment group and control group before and after treatment for 4 weeks.The overall ef-ficacy was compared between the treatment group and the control group,and serum IGF-1 levels were compared between the three groups.SPSS22.0 software was used to perform sta-tistical analysis on the data.result:1.Comparison before treatment:There was no significant difference in age,sex,duration of disease,number of macular edema,BCVA,macular thickness(CMT),and serum IGF-1 levels between the two groups(P>0.05).The serum IGF-1 levels in the treatment and control groups before treatment were significantly different from those in the normal control group(P<0.05);there was no significant difference in age and gender between the two groups of cases and the normal control group(P>0.05).2.Comparison of BCVA before and after treatment:The BCVA of both groups in-creased after treatment.There was a significant difference between the two groups before and after treatment(P<0.05).The difference in BCVA between the two groups after treatment was statistically significant(P<0.05).The BCVA improvement in the treatment group was better than that in the control group.3.Comparison of retinal hemorrhage and exudation after treatment:the retinal hemor-rhage and exudation of the two groups after treatment were statistically significant(P<0.05).The absorption of retinal hemorrhage and exudation was better in the treatment group than in the control group.4.Comparison of FFA changes after treatment:FFA changes after treatment in the two groups were statistically significant(P<0.05).The vascular leakage,micro hemangioma,non-perfused area,and new blood vessels in the treated group were.better than those in the control group.5.Comparison of macular central retinal thickness(CMT)after treatment:After treat-ment,both groups had decreased CMT after treatment,and the differences were statistically significant.Significance(P<0.05);There was a significant difference in CMT between the two groups after treatment(P<0.05).The decrease in CMT was better in the treatment group than in the control group.6.Comparison of overall curative effect:Both groups were effective after treatment(the effective rate in the treatment group was 86.3%,and the effective rate in the control group was 70.8%).The markedly effective,effective,and ineffective eyes of the two groups after treatment were statistically significant(P<0.05),the overall efficacy of the treatment group was better than that of the control group.7.Comparison of serum IGF-1 levels after treatment:There was a significant difference between the treatment group and the control group before the treatment compared with the normal control group(P<0.05);The serum IGF-1 levels in the treatment group and control group were lower than those before treatment.The difference between the two groups before and after treatment was statistically significant(P<0.05).There was no significant difference between the control group before and after treatment(P>0.05).After treatment,there was a significant difference between the treatment group and the control group compared with the normal control group(P<0.05).The was statistically significant difference between the treat-ment group and the control group after treatment(P<0.05),The content of-1 in the treatment group was higher than that in the normal control group.The decrease of serum IGF-1 in the treatment group was better than that in the control group.conclusion:1?Compared with laser photocoagulation alone,BuyangHuanwu Decoction combined with laser treatment is better for patients with qi-yin deficiency and typhoid-induced diabetic retinopathy.It can better improve the fundus micro hemangioma,vascular leakage,no perfu-sion vision in the affected eye and promote retinal tissue hemorrhage.Absorption and a area,and neovascularization,and reduce CMT.2?Pretreatment serum IGF-1 levels in patients with DR were significantly higher than those in healthy individuals,suggesting a certain relationship between serum IGF-1 and the occurrence and development of DR.After treatment,serum IGF-1 levels in the treatment group were significantly lower than before treatment.Although the serum IGF-1 level in the control group showed a decreasing trend,there was no significant difference between the treatment group and the treatment group.The serum IGF-1 level in the treatment group after treatment was similar to that in the control group.With a significant reduction,combined with;the results of efficacy evaluation,traditional Chinese medicine combined with laser treatment is more effective than laser alone,which may be achieved by regulating the initiation and ex-pression of IGF-1 mediated pathways in DR patients. |
