To analyze the status of satisfaction of analgesic effect in patients undergoing transurethral resection of the prostate(TURP)through questionnaire survey,and disuss the influencing factors of pain related to the patients.Based on the influential factors,we constructed the strategy for painless nursing program and mind map.The combined intervention was given to patients undergoing postoperative transurethral resection of the prostate to analyze the effect of the joint intervention,in order to enhance patients' awareness of participating in postoperative analgesia self-management initiatively and improve the effect of postoperative pain control,and then finally increase the satisfaction of postoperative analgesia.The ultimate goal of this study was to improve the quality of TURP nursing and provide a pre-foundation for the incorporation of mind map into clinical intervention programs.MethodsThrough the literature research,this study formulated a questionnaire that affected the degree of satisfaction with the analgesic effect in postoperative TURP patients,and consulted with urological specialists in the form of a questionnaire to determine the contents of the questionnaire including doctor-patient communication,anxiety and stress and other negative emotions,surgical importance,precautions before and after surgery.The 495 cases of TURP patients in our hospital qualified the inclusion criteria were surveyed.The survey data were analyzed by univariate and multivariate Logistic regression analysis,and then built the strategy for painless nursing program and mind map.135 cases of routine nursing care after TURP were selected as control group with painless nursing intervention,and 135 cases of painless nursing intervention combined with mind map as intervention group by convenient sampling method.We evaluated the effect of combined intervention in VAS score,PSQI score,SAS score,SDS score,PCA usage,ICI-Q-SF score and patient satisfaction.Results1.Factors affecting pain in TURP patients include: doctor-patient communication,anxiety and stress and other negative emotions,correctly evaluated the degree of pain,the use of analgesia pump,non-drug therapy to relieve pain,sterile operation method to prevent postoperative infection,postoperative guidance,knowledge of rehabilitation activity,the harm of postoperative pain and the history of pain education.2.The results showed that postoperative pain was significantly improved in the intervention group.The intervention group was lower than the control group at 6h,24 h,48h and 72 h after operation in terms of pain grade and score(VAS score)(P<0.05).There was a statistically significant difference in VAS scores between different time points and different Interventions,influenced by the factors of time and group(P<0.05).3.Compared with the control group,pittsburgh sleep quality index scale(PSQI)of intervention group was statistically significant in sleep quality,sleep time,sleep efficiency and sleep disorders(P<0.05).4.The results of anxiety self-rating scale(SAS),depression self-rating scale(SDS)score showed that Intervention group was significantly lower than the control group(P<0.05).5.In PCA consumption,the consumption of intervention group was significantly lower than that of control group at 6h,24 h,48h and 72 h after operation(P<0.05).There was a statistically significant difference in PCA consumption between different time points and different Interventions,influenced by the factors of time and group(P<0.05).6.ICI-Q-SF score showed that the score of intervention group was lower than that of control group(P<0.05).7.The satisfaction of the intervention group on analgesic effect was significantly higher than that of the control group(P<0.05).Conclusions1.Factors affecting pain in TURP patients include: doctor-patient communication,anxiety and stress and other negative emotions,correctly evaluated the degree of pain,the use of analgesia pump,non-drug therapy to relieve pain,sterile operation method to prevent postoperative infection,postoperative guidance,knowledge of rehabilitation activity,the harm of postoperative pain and the history of pain education.2.The application of mind mapping to implement painless nursing intervention can effectively increase the patient's enthusiasm and subjective initiative in pain management,improve the effect of perioperative pain control,and enhance patient satisfaction.
