Analysis Of Curative Effect Of Flurbiprofen For Spastic Pain After TURP

Objective: To analyze the curative effect of flurbiprofen for spastic pain and quality of life after TURP. And to annlyze the safety and efficacy of TURP and transurethral holmium laser enucleation of the prostate preliminarily.Method: Through retrospective analysis of 160 patients who were treated in our hospital from October 2014 to December 2015, Age 56-90 years old, the median age is 65 years old. We account the current treatment methods with the patient’s family in detail, the patients were randomly divided into four groups according to their wishes. In A group, the 38 patients were given flurbiprofen 100 mg mixture of 0.9% sodium chloride infusion 100 ml at the end of Ho LEP.In B group,the 40 patients were give were given flurbiprofen 100 mg mixture of 0.9% sodium chloride infusion 100 ml at the end of RURP.In C group,the 40 patients were give tramadol hydrochloride 100 mg mixture of 0.9% sodium chloride infusion 100 ml at the end of TURP.In D group,the 42 patients were give 0.9% sodium chloride infusion 100 ml at the end of TURP. Observe RU、Qmax、VAS score(1h、2h、4h、8h、12h、24h after operation)、toxicity and adverse effect of the drug、QOL、IPSS and record the frequency and duration ofbladder spasm in each group within 24 hours after operation.And observe the postoperative complications of A and B group, such as low sodium, urinary incontinence, urethral stricture, retrograde ejaculation. At the same time, to analyze and stufy the clinical effect and safety of TURP and Ho LEP preliminarily.Result: 1、There had no significant difference in age, body weight, prostate volume, operation time, anesthesia time and the heart rate, blood pressure, respiratory rate, blood oxygen saturation at each time of the four groups, there had no statistical significance(P > 0.05).2、In terms of the analgesic effect of drugs,the vas score at each time、the frequency and duration of bladder spasm in A、B、C group were less than those in D group,and there has no aignificant difference in A、B and C group,compared with the C group, there has statistically significant difference(P < 0.05), especially in the incidence of nausea and vomiting, there has statistically significant difference(P < 0.01).4、To compare TURP and Ho LEP, the operation time of B group patients who treated with TURP is 25-115 min, average time is 55 min, the removal of the gland tissue is 30-110 g, the average weight is 55 g. The operation time of B group patients who treated with Ho LEP is 31-96 min, average time is 50 min, the removal of the gland tissue is 36-125 g, the average weight is 59 g. there has no statistically significant difference in the weight of the resected prostate tissue and the time of surgery(P > 0.05). But in terms ofpostoperative complications, such as low sodium, urinary incontinence, urethral stricture, retrograde ejaculation, A group was lower than B group, there has statistically significant difference(P < 0.05).5、The maximum urinary flow rate(Qmax) and residual urine volume(RU) has significant difference in four groups before operation and at six months after operation. But there was no significant difference between the four groups in terms of maximum urinary flow rate(Qmax), residual urine volume(RU), quality of life score(QOL), and prostate symptom score(IPSS).Conclusion:Flurbiprofen can effectively decrease the recurrence rate of bladder spasm after TURP, relieve the pain of patients, and then reduce secondary bleeding, improve the success rate of surgery. It is a safe and effective way to prevent and treat bladder spastic pain after TURP operation. At the same time, Ho LEP has less post operative complications than TURP,and the clinical effect of them is considerable,it is valuable for clinical application.
