Objective The research aimed to summarize the distribution law and influencing factors of traditional Chinese medicine(TCM)syndrome types of Crohn's disease,and explore the clinical characteristics of its main syndromes and the characteristics of treatment and medication,and understand the relationship between the occurrence and development of Crohn's disease and the distribution of TCM syndromes.Finally,the research provided an objective basis for standardizing the TCM syndrome differentiation of the disease and explored the characteristics of its medication to provide the basis for individualized drug use to standardize the clinical syndrome differentiation and treatment of Crohn's disease.Methods The questionnaire was drawn up.And the characteristics of TCM syndromes,the law of syndrome differentiation and the rule of common Chinese medicine were observed by investigating the patients with Crohn's disease in Jiangsu Provincial Hospital of TCM.The correlation between the disease and age,occupation and classification was analyzed.The data were imported into SPSS 22.0 software package,and the original bedside database was established and analyzed statistically.The counting data were checked by chi-square test,and measured data:all groups were normal distribution,single factor analysis of variance,non-normal distribution using non-parametric test.There was statistical significance with P value less than 0.05.Results 60 cases of the disease were analyzed and studied.?Analysis of TCM syndromes:There are 7 main TCM syndromes which were intestinal dampness and heat(23 cases),dampness and blood stasis(11 cases),Qi and blood deficiency(8 cases),liver stagnation and spleen deficiency(7 cases),spleen and kidney yang deficiency(5 cases),spleen deficiency and dampness accumulation(4 cases),Yin and essence deficiency(2 cases).?Analysis of the age in patients:There was a wide distribution of age in patients which were divided into mental workers(65%),manual workers(16.7%),retirement(11.6%),students(6.7%).?Analysis of the occupations in patients:The occupation distributions were mental workers(65%),manual workers(16.7%),retirement 11.6%),students(6.7%).?Analysis of disease stages:Active stage accounted for 15 cases,and remission stage accounted for 45 cases.In the disease grading,mild patients were the maximum,and there were few of moderate and severe patients.?Analysis of Montreal classification:Most of patients were at A2 stage(17-40 years old)(61.67%).The most common location was L3(ileocecal type)(63.3%),and the disease behavior was narrowed(23.3%).The symptoms in digestive tract mainly were abdominal pain(35%)and diarrhea(35%).And the symptoms in parenteral mainly were crissum lesion(23.3),arthropathy(25%)and oral ulcer(20%).Complications of intestinal obstruction,intestinal perforation and abdominal abscess required surgical treatment which were not familiar in Gastroenterology dept.?Analysis of the general treatment of Western medicine:At present,the traditional drug treatment mode of Crohn's disease is ladder therapy.In this study,58 patients with Crohn's disease were treated by Western medicine,including aminosalicylic acid preparation(58 cases),immunosuppressant(45 cases),hormone(32 cases),biological agent(20 cases)and surgical treatment(8 cases).Intensive treatment with drug combination was used commonly in clinical treatment.The aminosalicylic acid combined with immunosuppressive agents treatment receivers were 20 cases accounting for the most.?Analysis of TCM Syndrome types and related factors:The distribution of syndromes under the age of 16 were significantly different(P<0.05),and was mainly in the deficiency of yin essence.In the age of 70 and over,the distribution of syndromes were significantly different(P<0.05),and mainly was spleen and kidney yang deficiency syndrome.The course of disease was significantly different from that of syndromes type over 5 years(P<0.05),and Qi and blood deficiency syndrome was the main factor.There was no significant difference in the distribution of different syndromes in occupation,sex and disease stages(P>0.05)?Analysis of common Chinese medicine:48 kinds of Chinese medicine totally were adopted,and the most frequently used medicines were Scutellaria baicalensis,fried paeoniae Alba,Radix Paeoniae Alba,fried Atractylodes macrocephala,Poria cocos,Ulmus pumila,Codonopsis pilosula,Coptis chinensis,stir-fried barley seed,old peel,fried Fructus auranti and so on.The Chinese medicine with strong antipyretic and detoxification is closely related to the location of the disease and often found in the ileocecal part and the small intestine.Conclusion Intestinal damp-heat syndrome,dampness and stasis syndrome,spleen deficiency and dampness accumulation syndrome,liver stagnation and spleen deficiency syndrome,qi and blood deficiency syndrome,spleen and kidney yang deficiency syndrome and yin and essence deficiency syndrome are the seven main syndrome types of Crohn's disease,in which intestinal damp-heat syndrome is the most common.The pathological factors of Crohn's disease include dampness,heat,toxin and blood stasis,and damp-heat runs through the development of the disease.Crohn's disease is located in the gastrointestinal tract,and spleen,liver,kidney,lung and many other internal organs related.Clinical treatment based on syndrome differentiation should taken into account the difference of five viscera and age,strengthen the spirit and dispel evil,and regulate the cold and heat.The selection of TCM should start with syndrome differentiation and syndrome differentiation to restore the function of viscera organs.
