Research backgroundAcne is a cutaneous appendage disease that occurs in the hair follicle sebaceous glands.It occurs at puberty and is characterized by pimples,pustules,nodules,cysts,and scarring,and it's often associated with septic spills.The incidence of acne is mainly related to four major causes such as excessive androgen and sebum,hyperkeratosis at the opening of the follicle sebaceous gland,acne actinomycete infection and secondary inflammatory reactions,some of them are also related to genetics,immunity,endocrine disorders,emotions,eating habits and other factors.Isotretinoin,also known as 13-cis-vitamin A acid,it has the function of inhibiting the secretion of sebaceous glands,regulating the keratinization of the sebaceous gland duct of the hair follicle,improving the anaerobic environment of the hair follicle,inhibiting the propagation of Propionibacterium acnes,anti inflammation and preventing the formation of scar formation.It plays a key role in the pathophysiology link of acne and is the most effective anti acne drug at present.However,isotretinoin can reduce the secretion of sebum,damage the function of the skin barrier,make the skin water loss increase and the moisture content of the epidermis decrease,so most patients will appear skin mucous membrane drying.Therefore,the patient's compliance is greatly reduced.Gamma-linolenic acid(GLA)is an unsaturated fatty acid in omega 6,which has the effect of eliminating inflammation and improving skin barrier.Recently,people have been told that 5-alpha reductase also has the function of inhibiting androgen,which can not only relieve the skin and mucosa desiccation caused by isotretinoin,but also has therapeutic effect on acne.Research purposeThe purpose of this study was to investigate the clinical efficacy of GLA in the prevention of skin and mucosal dryness in the treatment of acne vulgaris with isotretinoin and efficacy of the improvement of skin injury caused by acne.Research method From January 2016 to June 2017,the acne patients under the age of 18 and above40 years old were classified according to the Pillsbury clinical grading system of acne in the Department of Dermatology,GIDOK Hospital of.21 patients with grade above? and below grade ? were selected as subjects.The patients were divided into an experimental group of 11 people who had been given isotretinoin and gamma-linolenic acid,and a control group of 10 people who took only isotretinoin.Research result1.The skin moisture content of the control group decreased,and the TE WL in the experimental group decreased significantly compared with the control group.(p?0.016,0.015,0.011,0.016)2.Compared with the control group,the lip/skin dryness VAS score decreased significantly in the experimental group.(p = 0.019,0.033)3.The number of acne lesions decreased significantly in both groups,and the two groups only had differences in the number of papules,(p = 0.044)4.The decrease of Pillsbury level and the improvement of patients' satisfaction in the experimental group were significantly higher than that in the control group.(P =0.038,P = 0.035)Research conclusion1.This study found that GLA could be used effectively to reduce dryness caused by the treatment of acne patients with isotretinoin.Moreover,the additional improvement of acne lesions was observed when GLA was treated simultaneously.2.This study shows that GLA can be used effectively to improve acne lesions and to reduce skin and mucosal xerosis.
