Construction And Application Of Evaluation Index System In Specialized Hospitals

Background Medical institution evaluation usually refers to the evaluation by an organization other than the medical institution itself,the purpose is to judge how well the institution meets the quality and management system standards.Hospital evaluation is a cyclical process.The entire process is patient-centered and regards to the medical institution as a whole unit.It aims to provide a standardized,normative and objective assessment of the medical institution's process and promote the continuous improvement of the hospital's work.At present,many countries and regions in the world have conducted hospital evaluations and medical quality certifications.The representative countries include the United States,Australia,Germany,and Japan.The common feature of these countries is that hospital assessment work is generally undertaken by a third-party organization,which is'third-party evaluation'.Medical institution evaluation of China first started in the 1980s.During the development and evolution in the past 30 years,the pattern has been maintained that health administrative department served as the main evaluation organization of medical institution evaluation.With the release of policy documents such as 'Hospital Evaluation Interim Measures' and 'Health China 2030' in recent three years as well as the practice of nongovernmental institutions in China,'third-party evaluation' has attracted more and more attention in domestic hospital evaluation.Its application value has been recognized on the theoretical level and tested on the practical level.Objective This study aims to explore the theoretical methods,evaluation criteria and application experience of hospital evaluation in domestic and abroad,then construct the evaluation index system of specialized hospitals out of 'third party' identity by universities,next,conduct empirical research to verify its reliability among five ophthalmology hospitals in Shandong Province,and discuss the evaluation plan of specialized hospitals suitable for China's national conditions.Provide reference for the specialized hospitals to understand their own strengths,also provide reference for universities who intend to conduct third-party hospital evaluations.Methods This study used a combination of quantitative and qualitative research methods.Firstly,literature research method was used to sort out the theoretical basis of hospital comprehensive evaluation and the research progress of hospital evaluation systems at home and abroad.Then,literature analysis&optimization method and brainstorming method were used to construct preliminary indicators for the evaluation index system of this study.Using Delphi method,focus group discussion method,selected topic group discussion method to select evaluation indicators,estimate index weights,and formulate evaluation criteria.Finally,questionnaire method was used to collect data.Comprehensive evaluation method was used to obtain and analyze the evaluation results,SWOT analysis method was used to analyze subject of evaluation.In statistical processing,Excel was used to carry out questionnaire coding,data entry and sorting,and SPSS 22.0 was used to calculate the degree of activity,authority,and coordination of experts,as well as the average scores and coefficient of variation for each indicator.Results This study consists two parts:construction and application of the evaluation index system.Construction part:based on the principle of evaluation index system construction,comprehensively consider the hospital scale,hospital management,hospital services,research and teaching in four aspects,and build an evaluation framework including 4 first-level indicators,12 second-level indicators and 54 third-level indicators.The comprehensive evaluation index system of the hospital,applying the expert rating method to determine the weights and combination.weights of indicators at all levels,making reference to the domestic official standards or documents and related theories to study and formulate the evaluation criteria,then form a complete evaluation index system including the evaluation indicators,index weights and evaluation criteria.Application part:insufficient data hinders the comprehensive evaluation of four dimensions,the comprehensive scoring method was used to apply the data of five ophthalmology hospitals in Shandong Province to some dimensions(hospital management,teaching and scientific research)for calculation,then obtain the corresponding ranking results.Conclusion This study comprehensively uses the literature review,expert consultation,group discussion and other research methods to build a comprehensive evaluation index system for ophthalmic hospitals,which has been proven with good reliability,authoritative expert results and representative selection of indicators.Expert consultation method and expert rating method adopted for selecting indicators and estimating weights are subjective and need to be improved.The formulation of the evaluation standard refers to the.domestically published standard documents and related theoretical research,the applicability is good.When applying index system,the low attention and cooperation of specialized hospitals bring a lot of resistance to data collection work,limited by the small sample size and poorer data integrity,it's unable to further verify the reliability of this index system.This index system can provide reference for specialized hospitals to understand their own strengths,also become an example for universities who intend to conduct third-party hospital evaluations,but its application value and promotion significance remain to be demonstrated.
