The Clinical Observation Of Yin Zhi Lotion In The Treatment Of Anal Fissure And Its Effect On VEGF

Objective: To observe the effect of Yin Zhi lotion on the symptoms of pain and hematochezia,the time limitation for remission of pain after stool,the anal canal resting pressure and the wound healing time in patients with anal fissure.We will discuss the mechanism of the clinical treatment of the anal fissure through observing the expression of vascular endothelial growth factor(VEGF)by immunohistochemistry in the pathological tissue of the anal fissure.Methods: The experiment collected 60 subjects with anal fissure in stage I and II and was randomly divided into treatment group and control group,30 cases in each group.The treatment group was treated with Yin Zhi lotion and the control group was treated with Jin Xuan hemorrhoid fumigating and washing powder.The two groups' courses of treatment were 7 days.The pain,hematochezia,remission time of post pain,anal canal resting pressure,wound healing time and overall efficacy of two groups were observed after 1,3,5 and 7 days.After 7 days of treatment,we would choose 5 cases of the patients requiring surgical treatment in the treatment group and the control group,and 5 cases of only surgical patients.Their pathological tissues were taken for histopathological and immunohistochemical analysis of VEGF.Results: After 1 day of treatment,the remission time of pain after stool in the treatment group was shorter than the control group,p=0.01,p<0.05.After 3 days of treatment,the statistical results of pain,hematochezia and remission time of post pain between them were respectively 0.01,p<0.05,p=0.03,p<0.05 and p=0.03,p<0.05.The results showed that the time of pain,hematochezia and remission time of post pain in the treatment group was shorter than the control group.After 5 days of treatment,the P values of pain,hematochezia and remission time of post pain were respectively 0.04?0.01?0.02,and all less than 0.05.It turned out that the effect of the treatment group was better than the control group in relieving pain,hematochezia and remission time of post pain.After 7 days of treatment,pain p=0.03,p<0.05,hematochezia p=0.03,p<0.05,the results indicate that the treatment group was better than the control group in alleviating symptoms of pain and hematochezia.While the two groups had no significant difference in the remission time of post pain,p=0.36,p>0.05.The comparison of 7 days before and after treatment of anal canal resting pressure,the treatment group can better reduce the anal canal resting pressure than the control group,p=0.02,p<0.05.The treatment group could promote the wound healing compared with the control group,p=0.04,p<0.05.The two groups of drugs could promote the expression of VEGF by the independent sample rank and test,and the treatment group was more capable to promote the expression of VEGF than the control group,p=0.041,P < 0.05.Conclusion: Yin Zhi lotion and Jin Xuan hemorrhoid fumigating and washing powder both can improve the symptoms of pain and hematochezia in patients with anal fissure in stage I and II.The effect of Yin Zhi lotion on the patients with blood heat intestinal dryness type in stage I and II is better than that of the control group.It can significantly improve the main symptoms of anus pain and hematochezia in patients with anal fissure,shorten the pain time after stool,reduce the anal canal resting pressure,shorten the wound healing time and significantly promote the expression of VEGF in anal fissure lesions.
