Objective: To investigate the correlation and clinical significance of elderly patients with COPD and patients with acute exacerbation of chronic obstructive pulmonary disease(COPD)and low T3 syndrome.Methods: Serum T3,serum T4,free T3(FT3),free T4(FT4)and thyroid stimulating hormone(TSH)levels in 169 patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)without thyroid disease were measured.T3,T4,TSH,FT3,FT4 in patients with COPD and 169 healthy subjects were compared as control group.Serum thyroid hormone levels were determined by chemiluminescence method.Results: Compared with the control group,TT3 and FT3 of AECOPD group and COPD group were significantly lower,TT4 and FT4 no significant changes,TSH star increased.The AECOPD group consisted of five groups: heart failure group,malignancy group,cerebral infarction group,pulmonary infection group and simple AECOPD group,AECOPD group,thyroid hormone levels in five groups,heart failure group,malignant tumor group,Compared with AECOPD group,TT3 and FT3 decreased significantly,TT4 and FT4 had no obvious change,TSH star was elevated in group and lung infection group.Of the 169 AECOPD patients,31 had abnormal thyroid hormone levels,28 of whom had low T3 syndrome,with the highest proportion of patients with cerebral infarction.The 28-day mortality rate was 25.8% in the 31 patients with low T3 syndrome,and 2.17% in the 138 non-low-T3 syndrome groups.Conclusion: COPD group had lower T3 value than normal physical examination group,which indicated that low T3 syndrome had correlation with COPD.The T3 value of COPD group in acute exacerbation stage was lower than COPD group,suggesting that T3 value was inversely proportional to the severity of COPD;AECOPD in patients with AECOPD is associated with a higher mortality rate than AECOPD in patients with low T3 syndrome,suggesting that T3 may be a prognostic indicator for AECOPD.
