[Objective] This paper by comparing the clinical efficacy of Yiguanjian Decoction and haloperidol in the treatment of postoperative delirium in elderly hip fracture,to explore the clinical applicability and safety of Yiguanjian in this disease,in order to play traditional Chinese medicine economy, low side effect. To supplement the understanding and treatment of delirium in Chinese medicine, and to enrich the theoretical material of traditional Chinese medicine in the treatment of surgical diseases.[Methods] From 2017 to 01,76 cases of elderly patients with hip fracture treated in our hospital were selected, according to the relevant standards, and finally got the results of the treatment of 65 cases of patients with hip fracture.By random grouping software will be divided into the treatment group(33 cases)and control group(32cases),treatment group was treated with Yiguanjian Decoction, the control group were treated with oral rehydration,Haloperidol Tablets treatment, control of blood pressure, correct water and electrolyte disorders, functional exercise guidance and other conventional treatment. Two groups of patients before treatment, third days, fifth days period were delirium severity score and TCM symptoms assessment;on the tenth day of all the patients were followed up and again unified assessment, through the indicators of the final two sets of data for statistical analysis,to make an objective evaluation of Yiguanjian Decoction in treatment of elderly patients hip joints in patients with postoperative delirium in the efficacy and safety of.[Results] The two groups of elderly hip surgery related data of patients with delirium were compared, the patients in the two groups in gender, age, type of fracture, surgical site, intraoperative blood loss, time interval from onset to treatment,TCM syndrome score and delirium severity score no significant statistical difference(P >0.05).The follow-up data of third days, fifth days and 10 days showed that consistent Decoction and haloperidol have a certain effect on postoperative delirium. The third day of treatment, the DRS-R-98 score of treatment group was higher than that of control group, but the control group than the treatment group in delirium severity improved;in improving the severity of delirium,Yiguanjian than haloperidol more sustainable and effective;the DRS-R-98 score of treatment group and TCM syndrome score lower than the control group, statistical analysis showed that all have difference(P < 0.05).After treatment, the total effective rate of the control group was 90.63%,the incidence of adverse reactions was 12.50%,the total effective rate of the treatment group was 96.97%,the incidence of adverse reactions was 3.03%,and the incidence of adverse reactions in the control group was higher than that in the treatment group. However, all the laboratory indexes of the two groups had no significant changes before and after treatment.[Conclusion] It has been proved that the clinical effect of the Decoction in the treatment of postoperative delirium in elderly patients with hip fracture is low,and it is more effective than haloperidol in reducing the severity of postoperative delirium. However, there was no significant difference between the two groups in shortening the course of disease.
