Objective: To observe the clinical effect and safety of haloperidol combined with standardized treatment or standardized treatment. Methods: a prospective study of critical care medicine department of the First Affiliated Hospital of Xinjiang Medical University(March 2015 to March 2016),patients on ICU for more than 24 hours of extracorporeal circulation after cardiac surgery has pulled out the tracheal intubation in patients with severe fuzzy evaluation method medical awareness(CAM-ICU)assessment,collection of delirium patients.Randomly divided 120 patients into haloperidol combined with standardized treatment group A standardized treatment group B,treatment group C experience into the group. The use of CAM-ICU,12 h evaluation scores of each time, and record the negative rate;age, gender, weight, and chronic health evaluation II acute physiology, delirium type,CPOT score,RASS score, duration of delirium,ICU treatment time, hospitalization time, postoperative complications, hospital discharge rate, clinical the data of 28 day mortality after operation. Results: there were no statistically significant, compared three groups of patients with general information between 12 h,24h,36 h,48h,60 H,72h,A group,B group,CAM-ICU scores of negative rates were more than C group, there was statistical significance(P<0.05),A group,B group had no difference in each other;72h,84 h,96h.108 h,120h,CAM-ICU scores of the three groups. No difference between the negative rate of delirium duration,ICU residence time, hospitalization time,A group,B group were shorter than those of C group,A group,B group. The CAM-S score of A group was lower than that of B group,C group,B group,C group had no difference. No difference between the hospital discharge rate of A group and B group were higher than those in group C High, after 28 day all-cause mortality rate of A groupwas lower than that of B group was lower than that of C group. The postoperative complications occurred in group A patients with bradycardia were more than B,C group, pulmonary infection and hypoxia in patients were significantly less than B,C group, the difference was statistically significant(P<0.05).Conclusion: haloperidol combined with standardized treatment and standardized treatment in the treatment of delirium, safety and effectiveness are better than empirical treatment, haloperidol combined with bundle therapy treatment and no treatment of cluster the difference in effectiveness, haloperidol combined with standardized treatment in security than the standardized treatment combined with haloperidol treated, standardized treatment may be a new method for the treatment of delirium.
