Construction Of The Risk Model For The First Symptom Type Of Moyamoya Disease

Objective:To study the type and risk factors of the first symptom of moyamoya disease,and to set up the risk model of the first symptom type of moyamoya disease.Methods:The study involved the data of 485 patients with moyamoya disease who visited to The Second Affiliated Hospital Of Nanchang University from May 2013 to May 2016.The diagnosis of their moyamoya disease is based on the reports of DSA/ craniocerebral MRA and CTA.The patients were divided into ischemic group and hemorrhage group.By comparing the gender,age,smoking,alcohol drinking,family history,body weight index,blood glucose,blood pressure,blood lipid,homocysteine,moyamoya stage,aneurysm and other factors of each group to analyze the first clinical symptoms and risk factors of moyamoya disease and to build a risk model.The results were analyzed by single factor analysis,Logistic regression analysis and ROC curve analysis.Rresults: The hemorrhage group: gender(male 139,female 163),average age(49.59 + 10.12);smoking(38 smokers,264 non-smokers);alcohol drinking(18 were drinkers,284 were non-drinkers);family history(1 case of cerebral infarction,7 cases of cerebral hemorrhage,3 cases of moyamoya disease,11 cases of hypertension);BMI(56 persons are obesity and 246 persons are normal people);Blood glucose(97 persons are hyperglycemia and 205 persons are normal people);Blood lipid(130 persons are hyperlipidemia and 172 persons are normal people);Blood pressure(133 persons are hypertension and 169 persons are normal people);Homocysteine(85 persons are hypercystinemia and 217 persons are normol people);Moyamoya disease stage(22 persons were stage?and ?,271 persons were stage ? and ?,9 persons were stage ? and ?);aneurysm(73 persons with aneurysms,229 persons without aneurysms).The ischemia group: gender(male 96,female 87),average age(52.63 + 11.32);smoking(47 smokers,136 non-smokers);alcohol drinking(21 were drinkers,161 were non-drinkers);family history(8 case of cerebral infarction,2cases of cerebral hemorrhage,0 cases of moyamoya disease,9 cases of hypertension);BMI(35 persons are obesity and 246 persons are normal people);Blood glucose(54 persons are hyperglycemia and 129 persons are normal people);Blood lipid(81 persons are hyperlipidemia and 102 persons are normal people);Blood pressure(110 persons are hypertension and 73 persons are normal people);Homocysteine(82 persons are hypercystinemia and 101 persons are normol people);Moyamoya disease stage(59 persons were stage ? and ?,94 persons were stage ? and ?,30 persons were stage ? and ?);aneurysm(8 persons with aneurysms,175 persons without aneurysms).Conclusions: 1.The first clinical symptom of adult moyamoya disease is cerebral hemorrhage,which is more common than cerebral infarction.2.The risk factors of hemorrhagic who has moyamoya disease were aneurysm and Suzuki staging.3.The protection factors of hemorrhagic who has moyamoya disease include blood pressure,smoking and hypercystinemia.4.The risk models of hemorrhagic were constructed: Logistic y=-0.636* blood pressure,-0.844* smoking,+2.303* aneurysm,+2.11* moyamoya disease stage(1),-0.148* moyamoya disease staging(2)-0.615*,homocysteine +0.037.5.The area under the ROC curve is 0.802,and it has good accuracy.
